【摘要】：目的探討替羅非班聯(lián)合氯吡格雷對(duì)老年急性冠脈綜合征(ACS)患者血小板功能、炎癥因子及心功能的影響。方法 116例老年ACS病例隨機(jī)分為觀察組(60例)和對(duì)照組(56例)。對(duì)照組在常規(guī)治療的同時(shí)給予氯吡格雷,觀察組在對(duì)照組的基礎(chǔ)上給予替羅非班。治療30 d后,觀察兩組血小板功能血栓烷(TX)B2、血小板2-顆粒膜蛋白(GMP)-140、血小板P-選擇素(P-selectin)及炎癥因子白細(xì)胞介素(IL)-6、腫瘤壞死因子(TNF)-α、C反應(yīng)蛋白(CRP)及心功能左室射血分?jǐn)?shù)(LVEF)、左室舒張末期內(nèi)徑(LVEDD)、左室收縮末期內(nèi)徑(LVESD)的變化。結(jié)果治療后,兩組TXB2、GMP-140、P-selectin、IL-6、TNF-α、CRP水平均明顯下降,且觀察組明顯低于對(duì)照組(P0.05);治療后,兩組LVEF明顯上升,而LVEDD、LVESD明顯下降,觀察組LVEF明顯高于對(duì)照組,LVEDD、LVESD明顯低于對(duì)照組(P0.05);兩組不良反應(yīng)比較無(wú)統(tǒng)計(jì)學(xué)意義(P0.05)。結(jié)論替羅非班聯(lián)合氯吡格雷治療老年ACS時(shí)抗血小板作用更強(qiáng),且抑制炎癥反應(yīng)、改善心功能更顯著。
[Abstract]:Objective to investigate the effects of tirofiban combined with clopidogrel on platelet function, inflammatory factors and cardiac function in elderly patients with acute coronary syndrome (ACS). Methods 116 elderly patients with ACS were randomly divided into observation group (n = 60) and control group (n = 56). The control group was given clopidogrel at the same time of routine treatment, and the observation group was given tirofiban on the basis of the control group. After 30 days of treatment, the changes of platelet functional thrombomane (TX) B2, platelet 2-granule membrane protein (GMP)-140, platelet P-selector (P-selectin), inflammatory factor (IL)-6, tumor necrosis factor (TNF)-偽, C-reactive protein (CRP) and cardiac function left ventricular ejection fraction (LVEF), left ventricular end-diastolic diameter (LVEDD), left ventricular end-systolic diameter (LVESD) were observed. Results after treatment, the levels of TXB2,GMP-140,P-selectin,IL-6,TNF- 偽 and CRP in the two groups were significantly decreased, and the levels of LVEF and CRP in the observation group were significantly lower than those in the control group (P 0.05). After treatment, LVEF in the two groups increased significantly, while LVEDD,LVESD decreased significantly. The LVEF in the observation group was significantly higher than that in the control group and LVEDD,LVESD was significantly lower than that in the control group (P 0.05). Conclusion tirofiban combined with clopidogrel has a stronger antiplatelet effect, suppresses inflammatory reaction and improves cardiac function in elderly patients with ACS.
