【摘要】：目的探討老年慢性心力衰竭貧血患者血清腫瘤壞死因子(TNF)-α、B型鈉尿肽(BNP)、細胞黏附因子(ICAM)-1水平與心室重構(gòu)的關(guān)系。方法選取565例老年慢性心力衰竭貧血患者為研究組,另選取586例老年慢性心力衰竭患者為對照組,檢測血清TNF-α、BNP、ICAM-1水平,評價兩組心功能,計算左心室質(zhì)量指數(shù)(LVMI)、平均室壁應(yīng)力(MWS)。對比研究組不同貧血程度及不同紐約心臟病協(xié)會(NYHA)心功能分級患者血清TNF-α、BNP、ICAM-1水平及LVMI、MWS。結(jié)果研究組血清TNF-α、BNP、ICAM-1水平、LVMI、MWS均高于對照組(P0.05)。中度貧血患者TNF-α、BNP、ICAM-1水平、LVMI、MWS高于輕度者(P0.05)。研究組NYHA心功能分級為Ⅳ級患者血清TNF-α、BNP、ICAM-1水平、LVMI、MWS均高于Ⅱ級和Ⅲ級,且Ⅲ級患者均高于Ⅱ級(均P0.05)。結(jié)論老年慢性心力衰竭貧血患者血清TNF-α、BNP、ICAM-1水平升高能夠促進心室重構(gòu)的發(fā)生和發(fā)展。
[Abstract]:Objective To study the relationship between serum tumor necrosis factor (TNF)-1, B-type natriuretic peptide (BNP), cell adhesion factor (ICAM)-1 and ventricular remodeling in elderly patients with chronic heart failure. Methods 565 elderly patients with chronic heart failure were selected as the study group, and the other 586 patients with chronic heart failure were selected as the control group. The levels of TNF-, BNP and ICAM-1 in serum were measured. The left ventricular mass index (LVMI) and the mean wall stress (MWS) were calculated. The levels of TNF-1, BNP, ICAM-1 and LVMI, MWS in serum of patients with different anemia and different New York Heart Association (NYHA) were compared in the study group. Results The levels of TNF-, BNP, ICAM-1, LVMI and MWS in the study group were higher than those in the control group (P0.05). The levels of TNF-1, BNP, ICAM-1, LVMI and MWS in patients with moderate anemia were higher than those of mild anemia (P0.05). The NYHA functional classification of the study group was the level of serum TNF-1, BNP, ICAM-1, LVMI and MWS in the grade IV patients, which were higher than those of grade II and grade III, and all the patients with grade 鈪，
