【摘要】：目的:觀察心臟收縮力調(diào)節(jié)(CCM)對慢性心力衰竭(心衰)兔心肌重構(gòu)的影響并探討其可能機制。方法:選取新西蘭大白兔30只,分為假手術(shù)組、心衰組、心衰+CCM組,各組均n=10。假手術(shù)組僅開胸,未行升主動脈套扎。心衰組、心衰+CCM組通過升主動脈根部套扎法建立兔慢性心衰模型,心衰+CCM組模型制作成功后給予4周的CCM治療。分別于實驗第12周末和(或)第16周末對各組實驗動物,采用超聲心動圖檢測心臟功能,Masson染色評價心肌組織纖維化及病理形態(tài)改變,采用酶聯(lián)免疫吸附法(ELISA)檢測血漿B型利鈉肽(BNP)水平,蛋白免疫印跡(Westernblot)法檢測心肌組織I型膠原蛋白、Ⅲ型膠原蛋白、基質(zhì)金屬蛋白酶(MMP)2、MMP9、MMP抑制因子(TIMP)1、半乳糖凝集素3蛋白表達水平。結(jié)果:(1)超聲心動圖結(jié)果:實驗第12周末與假手術(shù)組相比,心衰組、心衰+CCM組左心室收縮末內(nèi)徑(LVESD)、左心室舒張末內(nèi)徑(LVEDD)明顯擴大,左心室縮短率(LVFS)和左心室射血分數(shù)(LVEF)明顯下降(P0.05);實驗第16周末,與心衰組相比,心衰+CCM組的LVESD、LVEDD、LVEF、LVFS指標明顯改善(P0.05)。(2)病理學改變:Masson染色示與假手術(shù)組相比心衰組心肌組織膠原纖維含量明顯增加(P0.05),CCM可部分減輕心衰心肌組織的膠原纖維沉積(P0.05)。(3)實驗第12周末,與假手術(shù)組相比,心衰組、心衰+CCM組血漿BNP水平明顯升高(P0.05)。實驗第16周末,與心衰組相比,心衰+CCM組的血漿BNP水平下降,但仍高于假手術(shù)組(P0.05)。(4)Western blot法檢測結(jié)果:心衰組心肌組織中I型膠原蛋白、Ⅲ型膠原蛋白、MMP2、MMP9、TIMP1、半乳糖凝集素3的蛋白表達水平明顯高于假手術(shù)組(P0.05),心衰+CCM組心肌組織中I型膠原蛋白、Ⅲ型膠原蛋白、MMP2、MMP9、TIMP1、半乳糖凝集素3的蛋白表達水平較心衰組明顯下降(P0.05),但仍高于假手術(shù)組(P0.05)。結(jié)論:心臟收縮力調(diào)節(jié)可以改善慢性心衰兔心功能及心肌重構(gòu),其機制可能與下調(diào)心衰心肌組織中I型膠原蛋白、Ⅲ型膠原蛋白、MMP2、MMP9、TIMP1、半乳糖凝集素3蛋白的表達有關(guān)。
[Abstract]:Aim: to investigate the effect of cardiac contractility on myocardial remodeling in rabbits with chronic heart failure (CHF) by regulating (CCM) and its possible mechanism. Methods: thirty New Zealand white rabbits were divided into sham operation group and heart failure CCM group. In the sham operation group, thoracotomy was only performed and no ascending aorta ligation was performed. Heart failure group, heart failure CCM group by ascending aortic root ligation method to establish a rabbit model of chronic heart failure, heart failure CCM model was successfully made 4 weeks after CCM treatment.
