【摘要】：目的探討心臟磁共振電影成像在評價射血分數(shù)保留心力衰竭(HFpEF)左心室舒張功能中的作用。方法應用心臟磁共振成像獲取30例HFpEF患者和15例心功能正常者(對照組)的左心室長軸兩腔、三腔、四腔及系列短軸電影圖像,繪制左心室容積-時間曲線(VTC)及整體應變曲線,比較兩組峰值充盈速率及充盈量,以及整體縱向、徑向、周向峰值應變和應變率等參數(shù)。結(jié)果與對照組相比,HFpEF患者左心室重構(gòu)指數(shù)(LVRI)增加(0.86±0.15vs 0.73±0.08,P0.05),舒張早、晚期峰值充盈速率比(PFR1/PFR2)降低或倒置(0.74±0.14vs 1.43±0.34,P=0.001),早期充盈量(FV1)及其占總充盈量比例降低(26.24±9.74vs 31.30±5.17,0.59±0.05vs 0.69±0.03;P均0.05),左心室整體縱向峰值應變(GLS)降低[(-14.96±1.79)%vs(-20.96±0.84)%,P0.01],整體縱向、徑向、周向峰值應變率均降低(0.65±0.16vs 1.29±0.27,-2.23±0.71vs-3.80±1.13,1.02±0.14vs 1.77±0.63;P均0.05)。結(jié)論通過心臟磁共振獲得的LVRI、PFR1/PFR2、FV1和整體周向峰值應變率能夠較準確地反映HFpEF左心室舒張功能的改變,對于HFpEF診治具有較高的臨床價值。
[Abstract]:Objective to investigate the role of cardiac magnetic resonance imaging (Mr) in evaluating left ventricular diastolic function in patients with congestive heart failure (HFpEF). Methods Cardiac magnetic resonance imaging (Mr) was performed in 30 patients with HFpEF and 15 patients with normal cardiac function (control group). Left ventricular long axis images of two, three, four and a series of short axis films were obtained, and left ventricular volume-time curves (VTC) and global strain curves were drawn.
