【摘要】：目前我國高血壓患病率及并發(fā)癥增長迅速,已成為中國人群心腦血管疾病最重要的危險因素之一。高血壓的防治僅依靠醫(yī)院門診是不夠的,必須重視并加強社區(qū)人群預防、篩查和管理,改革和建立能提供長期穩(wěn)定社區(qū)人群的服務體系。全科醫(yī)學團隊干預是一種新型的、有效的社區(qū)干預模式,其高血壓社區(qū)健康管理模式在提高知曉率、生活方式改善、血壓達標率、靶器官保護等方面起著積極的作用。本文就全科醫(yī)學團隊式健康管理在高血壓患者中的應用現(xiàn)狀進行綜述。
[Abstract]:At present, the prevalence and complications of hypertension are increasing rapidly in China, which has become one of the most important risk factors for cardiovascular and cerebrovascular diseases in Chinese population. It is not enough to rely on hospital outpatient service for the prevention and treatment of hypertension. It is necessary to pay attention to and strengthen the prevention, screening and management of community population, and reform and establish a service system that can provide long-term stable community population. General practice team intervention is a new and effective community intervention model. Its hypertension community health management mode plays an active role in improving awareness rate, lifestyle improvement, blood pressure compliance rate, target organ protection and so on. This article reviews the application of group health management in patients with hypertension.
