【摘要】：目的探討鐵調(diào)素在慢性心力衰竭(簡稱心衰)發(fā)生、發(fā)展中的作用。方法選擇慢性心衰患者113例作為心衰組,同期查體健康者100例作為對照組,檢測兩組血清鐵調(diào)素水平,分析心衰組伴貧血與不伴貧血者心功能分級、血清鐵調(diào)素水平;隨訪1年,比較心衰組伴與不伴貧血者心源性死亡例數(shù),存活者與死亡者入院時血清鐵調(diào)素水平。結(jié)果心衰組與對照組血清鐵調(diào)素水平分別為(95.10±43.37)、(27.45±5.03)ng/m L,兩組比較P0.01;心衰組輕度心衰者(64例)血清鐵調(diào)素水平高于重度心衰者(49例),二者比較P0.01;伴貧血者(21例)低于不伴貧血者(92例),二者比較P0.05。心衰組血清鐵調(diào)素水平與心功能分級呈負相關(guān)(r=-0.258,P0.01)。隨訪1年,心衰組發(fā)生心源性死亡10例,其中伴貧血者5例(23.81%)、不伴貧血者5例(5.43%),二者比較P0.05。心衰組存活者(103例)與死亡者(10例)入院時血清鐵調(diào)素水平分別為(106.94±20.3)、(105.10±35.80)ng/m L,二者比較P0.05。結(jié)論慢性心衰患者血清鐵調(diào)素水平升高,其水平變化可能誘發(fā)心衰合并貧血,導(dǎo)致患者心功能下降、預(yù)后差。
[Abstract]:Objective to investigate the role of iron modulin in the pathogenesis and development of chronic heart failure (CHF). Methods 113 patients with chronic heart failure were selected as heart failure group and 100 healthy persons as control group. The number of cardiogenic deaths in patients with and without anemia and the levels of serum iron modulin in patients with and without anemia were compared after 1 year follow-up. Results the serum levels of iron modulin in heart failure group and control group were (95.10 鹵43.37), (27.45 鹵5.03) ng/m / L, respectively (P 0.01), while in mild heart failure group (64 cases) it was higher than that in severe heart failure group (49 cases) (P 0.01), and that in anemia group (21 cases) was lower than that in without heart failure group (21 cases). Patients with anemia (92 cases) were compared with each other (P 0.05). There was a negative correlation between serum iron levels and cardiac function grade in heart failure group (r = -0.258, P 0.01). After 1 year follow-up, 10 cases of cardiac death occurred in heart failure group, 5 cases (23.81%) with anemia and 5 cases (5.43%) without anemia. The serum levels of iron modulin were (106.94 鹵20.3), (鹵35.80) ng/m / L in patients with heart failure (n = 10) and (106.94 鹵20.3), () / (105.10 鹵35.80) ng/m / L in patients with heart failure (n = 10), respectively (P < 0.05). Conclusion the level of serum iron modulin in patients with chronic heart failure is increased, which may induce heart failure complicated with anemia, leading to the decrease of heart function and poor prognosis.
