【摘要】：目的評(píng)價(jià)經(jīng)皮機(jī)械血栓清除術(shù)(PMT)聯(lián)合髂靜脈支架植入一期治療急性下肢深靜脈血栓形成(DVT)的可行性、安全性及有效性。方法 2014年12月至2016年1月收治12例急性下肢DVT伴左側(cè)髂靜脈受壓綜合征(IVCS)患者,男3例,女9例,平均年齡(50.42±16.21)歲;均有左下肢腫脹、疼痛癥狀,其中中央型3例,混合型9例。所有患者術(shù)前均置入下腔靜脈濾器,術(shù)后取出。Angio Jet血栓清除導(dǎo)管PMT術(shù)、球囊擴(kuò)張和髂靜脈支架植入均在同期完成,復(fù)查造影有殘余血栓時(shí)保留鞘管,給予溶栓治療。術(shù)后1、3、6、12個(gè)月門診隨訪,彩色超聲和(或)下肢靜脈造影復(fù)查了解深靜脈及支架內(nèi)血流通暢情況。結(jié)果 12例患者均同期完成手術(shù),技術(shù)成功率100%。手術(shù)時(shí)間60~110 min,平均(96.25±14.32)min;血栓抽吸時(shí)間51~280 s,平均(199.92±74.89)s。血栓清除率Ⅲ級(jí)10例,Ⅱ級(jí)2例,臨床癥狀均得到緩解。除部分患者在左髂總靜脈球囊擴(kuò)張中有不同程度疼痛外,無(wú)肺栓塞、嚴(yán)重出血等并發(fā)癥發(fā)生。平均隨訪(5.58±2.75)個(gè)月,11例彩色超聲和(或)下肢靜脈造影提示深靜脈及髂靜脈支架內(nèi)血流通暢,1例腦星形細(xì)胞瘤手術(shù)患者復(fù)發(fā)。結(jié)論同期PMT聯(lián)合髂靜脈支架植入治療急性下肢DVT方法安全可行,初步臨床療效滿意。
[Abstract]:Objective to evaluate the feasibility, safety and efficacy of percutaneous mechanical thrombus removal (PMT) combined with iliac vein stent implantation in the treatment of acute deep venous thrombosis of the lower extremity (DVT). Methods from December 2014 to January 2016, 12 patients (male 3, female 9, mean age 50.42 鹵16.21) with acute lower extremity DVT with left iliac vein compression syndrome were treated. Mixed type 9 cases. All the patients were implanted with inferior vena cava filter before operation. PMT was removed after operation. Balloon dilation and stent implantation of iliac vein were performed simultaneously. The sheath tube was retained in the case of residual thrombus in contrast examination and thrombolytic therapy was given. The blood flow in deep vein and stent was evaluated by color ultrasound and / or lower extremity venography. Results all the 12 patients completed the operation at the same time, and the technical success rate was 100%. The operative time was 60 ~ 110 min, the average time was 96.25 鹵14.32 min, the time of thrombus aspiration was 51 ~ 280 s, the average time was 199.92 鹵74.89 s.
