【摘要】：目的:通過觀察原發(fā)性高血壓(EH)合并單純高尿酸血癥(HUA)患者,排除影響尿酸排泄的后天因素后,應(yīng)用坎地沙坦前后,血尿酸、24小時尿尿酸、尿酸清除率的變化,判定坎地沙坦對尿酸排泄的影響,以幫助高血壓合并高尿酸血癥患者正確選擇降壓藥物。方法:選擇河北大學(xué)附屬醫(yī)院近16個月原發(fā)性高血壓合并單純高尿酸血癥患者79例;所有受試者實(shí)驗(yàn)前后檢測空腹血糖、血脂(膽固醇、甘油三酯)、血肌酐、尿PH值、24小時尿蛋白定量、尿常規(guī)。實(shí)驗(yàn)前檢測腫瘤標(biāo)記物、甲功五項(xiàng)、體質(zhì)指數(shù),篩選上述指標(biāo)均在正常參考范圍,體質(zhì)指數(shù)在18.5-28之間的受試者。受試者實(shí)驗(yàn)期間均為低嘌呤飲食,飲水量2000ml/d,連續(xù)2周后,連續(xù)應(yīng)用坎地沙坦3周,測定用藥前后受試者血尿酸、24小時尿尿酸,計(jì)算尿酸清除率和校正尿酸清除率(尿量為2000ml標(biāo)準(zhǔn)的情況下計(jì)算得到的尿酸清除率)。結(jié)果:應(yīng)用坎地沙坦3周后,患者血壓水平下降,平均動脈壓[治療前(115.38±8.55)vs治療后(98.04±6.55)mmHg,P0.01],血尿酸水平較治療前無明顯變化[治療前(531.92±77.55)vs治療后(529.31±83.05)umol/L,P0.05],校正尿酸清除率較治療前無明顯變化[治療前(14.16±2.35)vs治療后(14.31±2.50)ml/min,P0.05]。結(jié)論:坎地沙坦可降低平均動脈壓,對尿酸排泄無明顯影響。
[Abstract]:Objective: to observe the changes of uric acid and uric acid clearance rate in patients with essential hypertension (EH) complicated with hyperuricemia after removing the acquired factors affecting the excretion of uric acid before and after application of candesartan. To determine the effect of candesartan on uric acid excretion in order to help hypertensive patients with hyperuricemia to select antihypertensive drugs correctly. Methods: 79 patients with essential hypertension and simple hyperuricemia were selected from Hebei University affiliated Hospital in recent 16 months. Fasting blood glucose, blood lipids (cholesterol, triglyceride, creatinine, urine PH value, 24 hours urine protein quantification, urine routine) were measured before and after the experiment. Tumor markers, five items of thyroid function, body mass index were detected before and after the experiment. The subjects whose body mass index (BMI) was between 18.5-28 were all fed a low-purine diet with drinking water of 2000ml / d. After two weeks of continuous treatment, they were given candesartan for 3 weeks. Serum uric acid was measured before and after treatment, and the uric acid clearance rate and the corrected uric acid clearance rate were calculated. Results: after 3 weeks of application of candesartan, the uric acid clearance rate was calculated when the urine volume was the standard of 2000ml. The patient's blood pressure level drops, Mean arterial pressure [before and after treatment, 115.38 鹵8.55)vs, 98.04 鹵6.55mm HgG, P0.01], blood uric acid levels did not change significantly compared with those before treatment [529.31 鹵83.05umolol / L] after treatment [531.92 鹵77.55)vs], corrected uric acid clearance did not change significantly compared with pre-treatment [14.16 鹵2.35)vs before treatment, 14.31 鹵2.50 ml / min P 0.05]. Lowering the mean arterial pressure, There was no obvious effect on uric acid excretion.
