【摘要】：目的從循證醫(yī)學(xué)的角度探究男性骨密度改變與冠狀動(dòng)脈粥樣硬化之間的關(guān)系。方法首先根據(jù)研究的目的制定檢索策略,利用Pubmed、Embase、Cochrane圖書館、中國生物醫(yī)學(xué)文獻(xiàn)數(shù)據(jù)庫(CBM)、維普信息資源系統(tǒng)、相關(guān)期刊論文(CNKI)、萬方數(shù)據(jù)庫等檢索截至目前已發(fā)表的相關(guān)文獻(xiàn),對(duì)符合納入標(biāo)準(zhǔn)的文獻(xiàn)進(jìn)行數(shù)據(jù)的提取,利用Review Manager 5.3Meta分析軟件對(duì)提取的數(shù)據(jù)進(jìn)行分析。結(jié)果本研究最終納入5篇文獻(xiàn),包含骨密度降低組(骨量減少和骨質(zhì)疏松)325名患者,對(duì)照組259名患者。Meta分析顯示5項(xiàng)研究之間的異質(zhì)性檢驗(yàn)結(jié)果為I~2=62%,Chi~2=10.56,P=0.03,說明各研究之間存有異質(zhì)性,因此采用隨機(jī)效應(yīng)模型進(jìn)行分析,合并后的效應(yīng)量為OR=1.79,95%CI為(0.79,4.03),P=0.16;經(jīng)靈敏度分析剔除1篇樣本量較小的研究后,異質(zhì)性檢驗(yàn)結(jié)果顯示I~2=35%,Chi~2=4.64,P=0.20,表明各研究之間的異質(zhì)性較小并且可以接受,因此采用固定效應(yīng)模型進(jìn)行分析,合并后的效應(yīng)量為OR=1.22,95%CI為(0.76,1.96),P=0.41,最后所得的合并效應(yīng)量與樣本量較小的研究刪除之前的合并效應(yīng)量相比,結(jié)果基本一致,說明此次Meta分析的結(jié)果比較穩(wěn)定。結(jié)論男性骨密度的降低與冠狀動(dòng)脈粥樣硬化之間沒有顯著的相關(guān)性。
[Abstract]:Objective to explore the relationship between changes in bone mineral density and coronary atherosclerosis in men from the perspective of evidence-based medicine. The China Biomedical Literature Database (CBMN), the Weip Information Resources system, the China Journal Full-text Database (CNKII), the Wanfang Database, and so on, have been used to retrieve the relevant documents published so far, and to extract the data from the documents that meet the inclusion criteria. Review Manager 5.3 Meta analysis software was used to analyze the extracted data. Results the study included 5 articles, including 325 patients with osteopenia and osteoporosis. Meta-analysis of 259 patients in the control group showed that the results of heterogeneity test among the five studies were: I ~ (2 +) ~ (2 +) ~ (2) (10. 56) P ~ (0.03), indicating that there was heterogeneity in each study, so the random effect model was used to analyze the heterogeneity. The combined effect was 1.79 / 95CI = 0.79 / 4.03P0.16.The heterogeneity test results showed that the heterogeneity was 24.64 / 0.20, which indicated that the heterogeneity between the two studies was small and acceptable. Therefore, a fixed effect model was used to analyze the heterogeneity. The combined effect was 1.2295 CI = 0.766 / 1.96 / P0. 41. The result of the combined effect was basically consistent with that before the deletion of the sample, and the result was similar to that before the deletion of the sample, and the result was similar to that before the deletion of the sample, and the result was similar to that before the deletion of the sample. Conclusion there is no significant correlation between the decrease of bone mineral density and coronary atherosclerosis in men.
