【摘要】:目的:1.了解神經(jīng)肽Y(Neuropeptide Y,NPY)及超敏C反應(yīng)蛋白(High sensitivity C-reaction protein,hs-CRP)在射血分?jǐn)?shù)下降的心力衰竭(Heart failure with reduced ejection fraction,HFREF)患者體內(nèi)血漿濃度的變化;2.探討血漿神經(jīng)肽Y、超敏C-反應(yīng)蛋白與射血分?jǐn)?shù)下降的心力衰竭患者左室射血分?jǐn)?shù)(Left ventricular ejection fractions,LVEF)的相關(guān)性。方法:1.選取50例射血分?jǐn)?shù)下降的心力衰竭患者納入實(shí)驗(yàn)組,于入院次日清晨空腹時(shí)抽取肘靜脈血用于測(cè)定血漿神經(jīng)肽Y和超敏C-反應(yīng)蛋白的水平,并于同日行心臟彩色超聲檢查測(cè)定左室射血分?jǐn)?shù);在治療7日后,再次抽取肘靜脈血測(cè)定血漿神經(jīng)肽Y和超敏C-反應(yīng)蛋白水平。2.納入同期體檢正常的50人作為健康對(duì)照組。結(jié)果:1.實(shí)驗(yàn)組治療前的血漿神經(jīng)肽Y和超敏C-反應(yīng)蛋白水平較對(duì)照組顯著升高(P0.05),且心功能越差,二者的濃度越高。2.實(shí)驗(yàn)組血漿神經(jīng)肽Y和超敏C-反應(yīng)蛋白水平在治療后較治療前明顯下降(P0.05)。3.左室射血分?jǐn)?shù)在實(shí)驗(yàn)組較對(duì)照組有顯著下降(P0.05)。4.射血分?jǐn)?shù)下降的心力衰竭患者血漿神經(jīng)肽Y和超敏C-反應(yīng)蛋白水平與左室射血分?jǐn)?shù)呈負(fù)相關(guān)(P0.05)。結(jié)論:1.神經(jīng)肽Y和超敏C反應(yīng)蛋白參與了射血分?jǐn)?shù)下降的心力衰竭發(fā)生發(fā)展過程,二者在心衰患者體內(nèi)濃度越高,則心功能越差。2.神經(jīng)肽Y和超敏C反應(yīng)蛋白與左室射血分?jǐn)?shù)具有一定的相關(guān)性,隨著二者水平的升高,心室排血功能減弱。3.聯(lián)合檢測(cè)神經(jīng)肽Y和超敏C-反應(yīng)蛋白能夠提高評(píng)估心衰患者心功能及病情狀況的準(zhǔn)確性。
[Abstract]:Objective: 1. to investigate the neuropeptide Y (Neuropeptide Y NPY) and high sensitive C reactive protein (High sensitivity C-reaction protein, hs-CRP) in heart failure decreased ejection fraction (Heart failure with reduced ejection fraction, HFREF) concentrations in the patients; 2. to investigate the plasma neuropeptide Y, high-sensitivity C-reactive protein and shot down C- the left ventricular ejection fraction in patients with heart failure ejection fraction (Left ventricular ejection fractions, LVEF) correlation. Methods: 1. selected 50 cases were decreased ejection fraction in patients with congestive heart failure in the experimental group, to the hospital the next morning when fasting cubital venous blood samples for determination of plasma neuropeptide Y and hypersensitive C- reactive protein level, and on the same day for echocardiography measured left ventricular ejection fraction; in 7 days after treatment, RE Extraction venous blood determination of plasma neuropeptide Y and high sensitive C- reactive protein level. 2. as a healthy control group, 50 people with normal physical examination were included in the same period. Results: 1.. The plasma levels of neuropeptide Y and high sensitivity C- reactive protein in the experimental group were significantly higher than those in the control group before treatment (P0.05), and the worse the cardiac function, the higher the two concentration. 2. the level of plasma neuropeptide Y and hypersensitive C- reactive protein in the experimental group was significantly lower than that before treatment (P0.05). 3. the left ventricular ejection fraction in the experimental group was significantly lower than that in the control group (P0.05). 4. the levels of plasma neuropeptide Y and hypersensitive C- reactive protein in patients with decreased ejection fraction were negatively correlated with left ventricular ejection fraction (P0.05). Conclusion: 1., neuropeptide Y and high sensitivity C reactive protein play a role in the development of heart failure with decreased ejection fraction. The higher the concentration of two in patients with heart failure, the worse the cardiac function. 2. neuropeptide Y and hypersensitive C reactive protein have a certain correlation with left ventricular ejection fraction. With the increase of the level of the two, the function of ventricular ejection is weakened. 3. the combined detection of neuropeptide Y and hypersensitive C- reactive protein can improve the accuracy of the assessment of cardiac function and condition in patients with heart failure.
