【摘要】：目的:探討營(yíng)養(yǎng)干預(yù)對(duì)重度潰瘍性結(jié)腸炎(UC)的治療價(jià)值。方法:回顧性分析36例重度UC病人資料,其中20例病人進(jìn)行了腸外營(yíng)養(yǎng)(PN)或腸內(nèi)營(yíng)養(yǎng)(EN)治療為觀察組,其余16例為對(duì)照組。比較兩組病人住院時(shí)間以及治療前后的營(yíng)養(yǎng)指標(biāo)、血清內(nèi)毒素、IL-4和外周血CD4+CD25+調(diào)節(jié)性T細(xì)胞(CD4+CD25+Treg)水平。結(jié)果:給予營(yíng)養(yǎng)干預(yù)的觀察組病人住院時(shí)間較對(duì)照組明顯縮短(P0.05);治療后的血清總蛋白(TP)、清蛋白(ALB)、前清蛋白(PA)、轉(zhuǎn)鐵蛋白(TF)水平較治療前有明顯增高,且改善程度顯著高于對(duì)照組(P0.05);治療后的血清內(nèi)毒素水平亦較治療前明顯降低;IL-4及外周血CD4+CD25+Treg水平明顯增加,與對(duì)照組比,差異有統(tǒng)計(jì)學(xué)意義(P0.05)。在營(yíng)養(yǎng)支持中腹瀉的發(fā)生率為20%,占所有不良反應(yīng)的66.7%。結(jié)論:營(yíng)養(yǎng)支持治療可顯著改善重度UC病人的臨床病情及營(yíng)養(yǎng)狀況,對(duì)UC病人的免疫調(diào)節(jié)功能也有一定影響。
[Abstract]:Objective: to explore the value of nutritional intervention in the treatment of severe ulcerative colitis (UC). Methods: the data of 36 patients with severe UC were retrospectively analyzed. 20 cases were treated with parenteral nutrition (PN) or enteral nutrition (EN) as observation group and the other 16 cases as control group. The duration of hospitalization, nutritional parameters, serum endotoxin, IL-4 and peripheral blood CD4 CD25 regulatory T cell (CD4 CD25 Treg) levels were compared between the two groups. Results: the hospitalization time of the patients in the observation group was significantly shorter than that in the control group (P0.05). The level of serum total protein (TP), albumin (ALB), prealbumin (PA), transferrin (TF) was significantly higher than that before treatment and the improvement was significantly higher than that of the control group (P0.05). After treatment, the level of serum endotoxin was also significantly lower than that before treatment, IL-4 and peripheral blood CD4 CD25 Treg levels were significantly increased, compared with the control group, the difference was statistically significant (P0.05). The incidence of diarrhea in nutritional support was 20, accounting for 66.7% of all adverse reactions. Conclusion: nutritional support therapy can significantly improve the clinical condition and nutritional status of patients with severe UC, and also affect the immunomodulation function of UC patients.
