【摘要】：目的探討紐扣電池所致兒童食管損傷的臨床特征及治療方法,旨在提高兒童紐扣電池食管異物的診斷和治療水平。方法回顧性分析15例紐扣電池食管異物患兒的臨床特征、治療方法及預(yù)后。結(jié)果硬質(zhì)食管鏡取出紐扣電池12例,電子胃鏡下取出紐扣電池3例。12例患兒發(fā)生比較嚴重的食管損傷,10例患兒通過鼻飼、抗炎及支持治療得到完全恢復(fù)。2例術(shù)后發(fā)生食管穿孔,其中1例并發(fā)食管氣管瘺。結(jié)論紐扣電池嵌頓在食管容易造成嚴重食管損傷,處理關(guān)鍵是盡快確診。一旦確診紐扣電池嵌頓在食管,必須將其取出。紐扣電池在X線片上有比較明顯的特征,胸片可作為誤吞紐扣電池診斷首選檢查。如能根據(jù)病情選擇合適的手術(shù)方式及恰當(dāng)?shù)男g(shù)后處理,多數(shù)預(yù)后良好,但要警惕食管穿孔及食管氣管瘺等嚴重并發(fā)癥出現(xiàn)。
[Abstract]:Objective to investigate the clinical features and treatment of esophageal injury caused by button battery in children, in order to improve the diagnosis and treatment of esophageal foreign body.Methods the clinical features, treatment and prognosis of 15 cases of button-battery esophageal foreign body were retrospectively analyzed.Results 12 cases of button cells were removed by rigid esophagoscope, 3 cases of button batteries were removed by electronic gastroscopy. 10 cases of esophageal injury occurred in 10 cases. 2 cases of esophageal perforation were completely recovered after nasal feeding, anti-inflammatory and supportive therapy.One case was complicated with esophagotracheal fistula.Conclusion Button battery incarceration is easy to cause severe esophageal injury in esophagus, and the key to treatment is to make a diagnosis as soon as possible.Once the button battery is diagnosed as incarcerated in the esophagus, it must be removed.Button battery has obvious characteristics on X-ray film, chest film can be used as the first choice for the diagnosis of button battery.If we can choose the appropriate operation method and appropriate postoperative treatment according to the condition, most of the patients have good prognosis, but we should be on guard against serious complications such as esophageal perforation and esophagotracheal fistula.
