【摘要】:目的探討聲輻射力脈沖(ARFI)技術(shù)和天門冬氨酸氨基轉(zhuǎn)移酶與血小板比值指數(shù)(APRI)評(píng)價(jià)家兔肝纖維化模型的診斷價(jià)值。 方法應(yīng)用硫代乙酰胺(TAA)對(duì)32只家兔以皮下注射的方法建立肝纖維化模型,8只家兔作為對(duì)照組。于首次注射后第4、8、12周分批應(yīng)用ARFI獲取剪切波速度(SWV),計(jì)算APRI,取其肝臟行病理肝纖維化分期。以病理檢查結(jié)果為金標(biāo)準(zhǔn),比較SWV和APRI指數(shù)對(duì)家兔肝纖維化分期的診斷價(jià)值。 結(jié)果共29只家兔造模成功,其中肝纖維化分期為S1、S2、S3和S4期的分別有10、8、7和4只。SWV值和APRI均隨纖維化程度的加重而增高(P<0.05)。SWV (r=0.724, P0.05)和APRI指數(shù)(r=0.555,P<0.05)與肝纖維化病理分級(jí)均存在相關(guān)性。SWV和APRI診斷家兔肝纖維化S≥1、S≥2、S≥3、S=4的ROC曲線下面積分別為0.92、0.81,0.87、0.78,0.82、0.76,0.89、0.79。SWV和APRI聯(lián)合診斷S≥3的AUC為0.84,敏感度為81.80%,特異度為73.10%,陽性預(yù)測(cè)值為56.23%,陰性預(yù)測(cè)值為90.48%。 結(jié)論ARFI技術(shù)與APRI可以定量評(píng)估家兔肝纖維化程度,以ARFI技術(shù)的價(jià)值較高。
[Abstract]:Objective to evaluate the diagnostic value of acoustic radiation pulse ARFI (ARFI) technique and aspartate aminotransferase / platelet ratio index (APRI) in the diagnosis of hepatic fibrosis in rabbits. Methods Hepatic fibrosis model was established in 32 rabbits by subcutaneous injection of acetamide thioacetamide (TAA) as control group. Shear wave velocities were obtained by ARFI in batches at the 4th week of the 8th week after the first injection. Pathological hepatic fibrosis stages were performed in the viscera. The results of pathological examination were taken as the gold standard. To compare the diagnostic value of SWV and APRI index in the stage of hepatic fibrosis in rabbits. Results A total of 29 rabbits were successfully established. Among them, the hepatic fibrosis stage of S1 / S2 / S3 and S4 were 10 / 8 / 7 and 4, respectively.
