【摘要】：目的探討可溶性髓樣細(xì)胞觸發(fā)受體1(sTREM-1)在自發(fā)性細(xì)菌性腹膜炎(SBP)患者中的表達(dá)情況。方法選取2013年6月—2014年12月紹興市人民醫(yī)院消化內(nèi)科收治的肝硬化腹腔積液患者,23例并發(fā)SBP的患者為SBP組,66例未并發(fā)SBP的患者為非SBP組,SBP組患者根據(jù)病情分為輕癥SBP(14例)和重癥SBP(9例)。檢測(cè)其血清白細(xì)胞計(jì)數(shù)(WBC)、C反應(yīng)蛋白(CRP)、降鈣素原(PCT)、sTREM-1水平及腹腔積液細(xì)胞、多形核白細(xì)胞計(jì)數(shù)(PMN)、乳酸脫氫酶(LDH)、sTREM-1水平。結(jié)果 SBP組血清WBC、CRP、PCT、sTREM-1及腹腔積液細(xì)胞、PMN、LDH、sTREM-1水平均較非SBP組升高(P0.05)。非SBP組腹腔積液培養(yǎng)均為陰性,SBP組細(xì)菌培養(yǎng)13例(56.5%)患者陽性。輕癥及重癥SBP患者血清WBC、CRP、PCT、sTREM-1及腹腔積液細(xì)胞、PMN、LDH、sTREM-1水平比較,差異均無統(tǒng)計(jì)學(xué)意義(P0.05)。Pearson相關(guān)性分析顯示,SBP組和非SBP組患者血清sTREM-1水平與腹腔積液sTREM-1水平呈正相關(guān)(r=0.737,P0.001;r=0.275,P=0.025)。結(jié)論肝硬化腹腔積液并發(fā)SBP患者血清和腹腔積液sTREM-1水平升高,但是不同病情程度患者無差異,sTREM-1有可能作為診斷SBP的參考指標(biāo)。
[Abstract]:Objective to investigate the expression of soluble myeloid cell trigger receptor (sTREM-1) in patients with spontaneous bacterial peritonitis (SBP). Among the 23 patients complicated with SBP, 66 patients without SBP in SBP group were divided into mild SBP(14 group and severe SBP(9 group according to their condition. The serum leukocyte count and the levels of serum WBC C reactive protein, procalcitonin TREM-1 and peritoneal effusion cells were measured. Results in SBP group, the levels of serum WBCG-CRP- PCTnm-1 and peritoneal effusion cells were significantly higher than those of non-#en1# group. The positive rate of bacterial culture in non-SBP group was 13 cases (56.5%), while that in non-#en2# group was negative (13 cases, 56.5%), compared with that in non-#en1# group (P < 0.05), and the level of TREM-1 was significantly higher than that in control group (P < 0.05), and the level of TREM-1 was significantly higher than that in control group (P < 0.01). The positive levels of TREM-1 in SBP group were significantly higher than those in control group (P < 0.05). Serum levels of TREM-1 in patients with mild or severe SBP were compared with those in peritoneal effusion cells. There was no significant difference between serum sTREM-1 level and sTREM-1 level in peritoneal effusion of patients with SBP. Conclusion the levels of sTREM-1 in serum and peritoneal effusion in patients with cirrhosis and peritoneal effusion complicated with SBP were significantly higher than those in control group and non-SBP group, and there was a positive correlation between the serum sTREM-1 level and the sTREM-1 level in peritoneal effusion. Conclusion there is a positive correlation between the level of serum sTREM-1 and the level of sTREM-1 in peritoneal effusion. Conclusion the level of sTREM-1 in serum and peritoneal effusion is increased in patients with cirrhosis and peritoneal effusion. However, there was no difference between patients with different severity of disease and serum TREM-1 might be used as a reference index for the diagnosis of SBP.
