【摘要】：目的應(yīng)用超聲測(cè)量肝硬化患者頸段胸導(dǎo)管內(nèi)徑,評(píng)估不同類型肝硬化患者胸導(dǎo)管內(nèi)徑的差異及意義。方法選取我院臨床確診為慢性肝病肝硬化患者163例,其中慢性乙型肝炎肝硬化(CHB)患者61例,慢性丙型肝炎肝硬化(CHC)患者32例,酒精性肝硬化(AC)患者42例,自身免疫性肝炎肝硬化(AHC)患者28例。同期選取65例健康志愿者作為對(duì)照組。應(yīng)用高頻超聲測(cè)量頸段胸導(dǎo)管內(nèi)徑(CTDd),比較各組CTDd的差異。結(jié)果 CHB、CHC、AC、AHC組頸段胸導(dǎo)管超聲顯示率分別為97%、100%、100%、89%,健康對(duì)照組顯示率為89%,顯示率差異無(wú)統(tǒng)計(jì)學(xué)意義(P0.05)。健康對(duì)照組的CTDd測(cè)值最小,為(2.11±0.58)mm;AHC組、CHB組、CHC組AC組CTDd測(cè)值分別為(3.96±1.31)mm、(4.49±1.31)mm、(4.62±1.56)mm、(6.07±2.60)mm,較對(duì)照組均明顯增寬,差異有統(tǒng)計(jì)學(xué)意義(P0.000);AHC組、CHB組、CHC組各組間差異無(wú)統(tǒng)計(jì)學(xué)意義(P0.05);AC組CTDd測(cè)值較AHC組、CHB組、CHC組增寬,差異有統(tǒng)計(jì)學(xué)意義(P0.01)。結(jié)論肝硬化患者胸導(dǎo)管內(nèi)徑較正常人明顯增寬,不同病因?qū)е略鰧挸潭扔兴顒e。
[Abstract]:Objective to measure the inner diameter of cervical thoracic ducts in patients with liver cirrhosis and to evaluate the difference and significance of the diameter of thoracic ducts in patients with different types of cirrhosis. Methods 163 patients with chronic liver cirrhosis were selected in our hospital. There were 61 patients with chronic hepatitis B cirrhosis, 32 patients with chronic hepatitis C cirrhosis and 42 patients with alcoholic cirrhosis. Twenty-eight patients with autoimmune hepatitis cirrhosis (CTDd) and 65 healthy volunteers (control group) were selected as control group. High frequency ultrasound was used to measure the inner diameter of cervical thoracic duct and compare the difference of CTDd in each group. The display rates were 97% and 100%, respectively. The display rate of the healthy control group was 89 and the difference was not statistically significant (P 0.05). The CTDd of the healthy control group was the smallest. The CTDd values of AC group in CHC group were 3.96 鹵1.31 鹵1.31 mm, respectively, which were significantly wider than those in control group (4.62 鹵1.56 鹵1.56). The difference was not statistically significant (P 0.000). There was no significant difference between the two groups. The CTDd value of AC group was significantly wider than that of AHC group (P < 0.05), and the value of CTDd in AC group was significantly wider than that in AHC group (P < 0.05), which was significantly higher than that in control group (P < 0.05). Conclusion the inner diameter of thoracic ducts in patients with liver cirrhosis is significantly wider than that in normal subjects, and the extent of enlargement is different in different etiology.
