【摘要】：目的評(píng)價(jià)同軸微切口白內(nèi)障超聲乳化術(shù)控制術(shù)中虹膜松弛綜合征(intraoperative floppy iris syndrome,IFIS)的治療效果。方法前瞻性隨機(jī)對(duì)照研究。2014年10月至2016年10月服用坦索羅辛≥2周的年齡相關(guān)性白內(nèi)障患者80例(80眼),隨機(jī)分為2組:微切口組和標(biāo)準(zhǔn)切口組,每組40例(40眼),分別通過(guò)1.8 mm微切口和2.6 mm標(biāo)準(zhǔn)透明角膜切口行同軸超聲乳化白內(nèi)障吸出聯(lián)合人工晶狀體植入術(shù)。記錄并對(duì)比兩組患者術(shù)中IFIS的發(fā)生率、嚴(yán)重程度及手術(shù)并發(fā)癥,術(shù)后1 d、1周、1個(gè)月的裸眼視力。結(jié)果術(shù)后1 d、1周、1個(gè)月的裸眼視力,微切口組分別為0.83±0.12、0.86±0.10、0.89±0.11,標(biāo)準(zhǔn)切口組分別為0.71±0.12、0.75±0.11、0.83±0.12,差異均有統(tǒng)計(jì)學(xué)意義(均為P0.05),微切口組裸眼視力均優(yōu)于標(biāo)準(zhǔn)切口組。微切口組IFIS的發(fā)生率為60.0%,標(biāo)準(zhǔn)切口組IFIS的發(fā)生率為82.5%,差異有統(tǒng)計(jì)學(xué)意義(P0.05)。兩組術(shù)中IFIS發(fā)生的嚴(yán)重程度比較差異有統(tǒng)計(jì)學(xué)意義(P0.05),微切口組IFIS發(fā)生的嚴(yán)重程度低于標(biāo)準(zhǔn)切口組。結(jié)論同軸微切口白內(nèi)障超聲乳化術(shù)對(duì)具有IFIS發(fā)生風(fēng)險(xiǎn)的白內(nèi)障患者來(lái)說(shuō)是一種安全有效的手術(shù)方式。
[Abstract]:Objective to evaluate the efficacy of coaxial microincision phacoemulsification in the control of iris relaxation syndrome (intraoperative floppy iris syndrome,IFIS). Methods A prospective and randomized controlled study was conducted. From October 2014 to October 2016, 80 patients (80 eyes) with age-related cataract were randomly divided into two groups: microincision group (n = 40) and standard incision group (n = 40). Coaxial phacoemulsification combined with intraocular lens implantation was performed through 1.8 mm microincision and 2.6 mm standard transparent corneal incision, respectively. The incidence, severity and complications of IFIS were recorded and compared between the two groups. The uncorrected visual acuity was 1 day, 1 week and 1 month after operation. Results on the 1st day, 1 week and 1 month after operation, the uncorrected visual acuity of the microincision group was 0.83 鹵0.12, 0.86 鹵0.10, 0.89 鹵0.11, and that of the standard incision group was 0.71 鹵0.12, 0.75 鹵0.11, 0.83 鹵0.12, the difference was statistically significant (all P 0.05). The uncorrected visual acuity of the microincision group was better than that of the standard incision group. The incidence of IFIS was 60.0% in the microincision group and 82.5% in the standard incision group, the difference was statistically significant (P 0.05). There was significant difference in the severity of IFIS between the two groups (P 0.05). The severity of IFIS in the microincision group was lower than that in the standard incision group. Conclusion phacoemulsification with coaxial microincision is a safe and effective method for cataract patients with IFIS risk.
