【摘要】：目的:探討鼓室內(nèi)灌注、明膠海綿經(jīng)圓窗給藥后豚鼠外淋巴液中甲潑尼龍琥珀酸鈉（methylprednisolone，MTH)的代謝動力學(xué)特征，為臨床選擇經(jīng)鼓室給藥方式提供實驗依據(jù)。 方法:采用鼓室內(nèi)灌注MTH (62.5mg/ml)及將浸滿MTH（62.5mg/ml）的明膠海綿放置于圓窗上兩種方式給藥,采集2種途徑給藥后1～12h鼓階外淋巴液,應(yīng)用高效液相色譜儀分別檢測其中的甲潑尼龍濃度。 結(jié)果:鼓室內(nèi)灌注或明膠海綿經(jīng)圓窗給藥后外淋巴液中MTH的最高濃度均出現(xiàn)在給藥后1.5h;在取材的每個時間點,鼓室內(nèi)灌注給藥后外淋巴液中的MTH濃度均高于明膠海綿經(jīng)圓窗給藥者,除5、6及12h外,兩者相比差異均有統(tǒng)計學(xué)意義。鼓室內(nèi)灌注給藥后外淋巴液中MTH的生物半衰期t1/2=1.84h,明膠海綿圓窗給藥后甲潑尼龍t1/2=1.87h。鼓室內(nèi)灌注、明膠海綿經(jīng)圓窗給藥后12h取材時間點的豚鼠較給藥前與取外淋巴前ABR波形Ⅲ波閾值比較差異均無統(tǒng)計學(xué)意義(P值均0．05)。 結(jié)論:局部給藥后MTH能夠有效地經(jīng)圓窗膜滲透進入外淋巴液。鼓室內(nèi)灌注給藥后外淋巴液中的甲潑尼龍濃度較高。該濃度的MTH對內(nèi)耳功能未見明顯影響。

[Abstract]:Objective: to investigate the metabolic kinetic characteristics of sodium methylprednisolone succinate (methylprednisolone,MTH) in perilymph of guinea pigs after indoor infusion of gelatin sponge and administration of gelatin sponge through round window, and to provide experimental basis for clinical selection of drug administration through tympanic cavity. Methods: MTH (62.5mg/ml) was infused in tympanic cavity and gelatin sponge soaked with MTH (62.5mg/ml) was placed on a round window. The perilymph of the tympanic stage was collected 1 h after administration, and the concentration of methylprednisolone was detected by high performance liquid chromatography (HPLC). Results: the highest concentration of MTH in perilymph was 1.5 hours after administration of gelatin sponge through round window, and the concentration of MTH in perilymph after intratympanic infusion was higher than that of gelatin sponge through round window at each time point, except for 5, 6 and 12 hours, there was significant difference between the two groups.
