羊膜移植治療眼化學燒傷及熱燒傷的臨床觀察

【摘要】:目的:眼化學燒傷及熱燒傷是我國常見的眼外傷,其損傷嚴重,傳統(tǒng)治療效果有限。羊膜因其獨特的作用一直是眼表重建過程中研究的熱點。本文觀察使用羊膜移植術(Amniotic membrane transplantation,AMT)治療眼化學燒傷、熱燒傷后的臨床治療效果,以提高對這一治療手段的認識。 方法:收集2009年09月—2011年10月我院收治的眼化學燒傷、熱燒傷患者22人(22眼),其中Ⅱ度5眼,Ⅲ度14眼,Ⅳ度3眼,對其施行AMT眼表重建術,觀察術后羊膜植片情況、角膜上皮修復情況、手術前后患者視力變化及術后排斥反應、角膜新生血管形成情況。術后隨訪6-18個月,平均12個月。 結果:術后早期羊膜植片輕度水腫,第3-5天起水腫減輕,1周后開始變薄,部分融解吸收。術后1個月,觀察不到羊膜組織存在。羊膜植片于術后一周內部分脫離者10眼,其中6眼角膜上皮或基質尚存在較大范圍缺損,故施行第2次羊膜移植術。22名患者的22眼中,20眼(Ⅱ度5眼,Ⅲ度14眼,Ⅳ度1眼)術后效果較好,表現為角膜刺激癥狀明顯減輕,上皮愈合、視力有所提高,手術前后視力對比P<0.05,未出現感染或排斥現象。其中4眼(Ⅲ度3眼,,Ⅳ度1眼)在首次羊膜移植后,因植片脫落、融解,眼表修復效果不滿意,經歷2次手術。2眼(Ⅳ度2眼)手術失敗,表現為經過2次AMT,仍出現角膜穿孔,最終行板層角膜移植術。最終共12眼(Ⅲ度9眼,Ⅳ度3眼)發(fā)生角膜新生血管:<1個象限者5眼(Ⅲ度5眼),1-3象限者5眼(Ⅲ度4眼,Ⅳ度1眼),>3個象限者2眼(Ⅳ度2眼)。 結論:1、對于中、重度眼化學燒傷、熱燒傷病例,AMT可以起到一定減輕癥狀,縮短病程,優(yōu)化預后的效果。對于極嚴重者可優(yōu)化眼表微環(huán)境,為進一步手術爭取時間。2、AMT術后早期羊膜脫落、融解,臨床效果不滿意時,可考慮多次移植以取得較穩(wěn)定的臨床效果。3、在重度病例,單純靠AMT無法完成眼表重建的全部過程。應把AMT看做眼表重建的一個步驟,待炎癥反應減輕,眼表環(huán)境好轉,需根據損傷情況進一步處置,以完成損傷后的眼表重建。
