納米銀制劑治療中重度真菌性角膜潰瘍的療效研究

【摘要】:目的:建立真菌性角膜潰瘍動(dòng)物模型,研究病灶清創(chuàng)聯(lián)合基質(zhì)內(nèi)注射納米銀制劑治療中重度真菌性角膜潰瘍的療效。方法:制備濃度為5x107菌落形成單位(colony forming units,CFU)/ml的標(biāo)準(zhǔn)煙曲霉菌液,選用成年日本大耳白兔60只,按隨機(jī)數(shù)字表法分為兩組并標(biāo)記,角膜基質(zhì)內(nèi)注射煙曲霉菌組(a組,n=54只),角膜基質(zhì)內(nèi)注射生理鹽水組(b組,n=6只),所有動(dòng)物均選用左眼為實(shí)驗(yàn)眼。a組用胰島素注射器(帶30號(hào)針頭),行左眼角膜基質(zhì)內(nèi)注射1u含標(biāo)準(zhǔn)煙曲霉菌的混懸液(形成一個(gè)直徑約2mm的菌斑),同法b組注射等量生理鹽水。接種后均于下方穹窿結(jié)膜注射4u(0.5mg)地塞米松磷酸鈉注射液。48小時(shí)后裂隙燈顯微鏡下檢查、10%KOH涂片、真菌培養(yǎng)及共焦顯微鏡檢查造模情況。配置250μg/ml、500μg/ml、750μg/ml三種濃度的納米銀溶液及500μg/ml伏立康唑溶液備用。選用符合標(biāo)準(zhǔn)的動(dòng)物模型54只兔隨機(jī)分成五組并標(biāo)記,250μg/ml的納米銀治療組(A組n=12只),500μg/ml的納米銀治療組(B組n=12只),750μg/ml的納米銀治療組(C組n=12只),500μg/ml伏立康唑溶液治療組(D組,n=12只),生理鹽水組(E組n=6只)。A—E組分別行角膜潰瘍清創(chuàng)及碘酊灼燒后,用胰島素注射器(30號(hào)針頭)行角膜基質(zhì)內(nèi)注射相應(yīng)的試劑,總量約4u。同法E組注射等量生理鹽水。建模成功后給藥頻率:10天前1次/3天、10天后1次/5天、20天后1次/7天。給藥之后裂隙燈顯微鏡下觀察角膜潰瘍愈合情況。治愈后的兔子繼續(xù)觀察3個(gè)月了解預(yù)后情況。統(tǒng)計(jì)軟件SPSS17.0用于統(tǒng)計(jì)描述與推斷,定性資料采用頻數(shù)及百分比表示,定量資料采用均數(shù)及標(biāo)準(zhǔn)差表示,定性資料的組間比較采用卡方檢驗(yàn),定量資料的組間比較采用單因素方差分析,組間兩兩比較采用最小差異顯著法(least significance of difference,LSD),統(tǒng)計(jì)學(xué)檢驗(yàn)標(biāo)準(zhǔn)為P0.05。結(jié)果:建模48小時(shí)后模型a組建模成功率100%,模型b組無角膜感染征象。經(jīng)過三種不同濃度的納米銀溶液(A、B、C組)、500μg/ml伏立康唑溶液(D組)治療煙曲霉菌性角膜潰瘍后得出,B組總有效率最高,與A、C、E組比較有統(tǒng)計(jì)學(xué)意義(P0.05),與D組比較無統(tǒng)計(jì)學(xué)意義(P0.05)。B組和A、C組達(dá)到有效的治療時(shí)間比較差異具有統(tǒng)計(jì)學(xué)意義(P0.05),與D組比較無統(tǒng)計(jì)學(xué)意義(P0.05)。治愈后繼續(xù)觀察3個(gè)月后均無復(fù)發(fā)。結(jié)論:角膜基質(zhì)內(nèi)注射煙曲霉菌液建立模型的成功率為100%。三種不同濃度的納米銀溶液中,500μg/ml的納米銀治療中重度煙曲霉菌性角膜潰瘍總有效率最高,達(dá)到有效的治療時(shí)間最短。500μg/ml的納米銀與伏立康唑溶液治療中重度煙曲霉菌性角膜潰瘍均有良好效果,達(dá)到有效的治療時(shí)間相近,未發(fā)現(xiàn)角膜毒性反應(yīng),且治愈后觀察3個(gè)月后均無復(fù)發(fā)。納米銀價(jià)格相對(duì)較低、不良反應(yīng)少、制備方便,有可能成為治療中重度真菌性角膜潰瘍的一種新藥。
