【摘要】：目的:對利多卡因靜脈注射、鼓室給藥、穴位注射等不同給藥途徑治療耳鳴的文獻進行綜合評價,評估利多卡因在耳鳴綜合治療中的應用價值。方法:檢索中國知網(wǎng)、萬方、維普、Pubmed、SciVerse ScienceDirect、Springer、OVID等數(shù)據(jù)庫,篩選出利多卡因治療耳鳴的文獻進行嚴格質(zhì)量評價和資料提取,采用Revman5.2軟件對文獻相關指標進行Meta分析。結(jié)果:共納入文獻16篇,1 203例患者,其中中文文獻14篇,英文文獻2篇。耳鳴患者病程7h~20年,療效的評估主要依據(jù)響度分級、嚴重程度量表或主觀感受。所有文獻均未具體提及療效維持時間,僅以"短期"、"較短"等詞加以說明。其中靜脈注射組133例,治療總有效率為73.4%(98例);鼓室給藥組50例,總有效率為74.0%(37例);穴位注射組332例,總有效率為87.7%(291例)。各對照組例數(shù)分別為125、40和326例,總有效率分別為42.4%、50.0%和58.3%。Meta分析結(jié)果顯示,利多卡因經(jīng)3種不同給藥方式治療耳鳴的總有效率均高于傳統(tǒng)藥物(P0.05)。結(jié)論:利多卡因不同給藥途徑盡管作用維持時間短,但均有一定耳鳴抑制作用。其作為耳鳴綜合治療的輔助措施,有助于縮短適應耳鳴的時間而非消除耳鳴,臨床應用中應注意分析。
[Abstract]:Objective: to evaluate the literature of lidocaine intravenous injection, tympanic administration and acupoint injection in the treatment of tinnitus, and to evaluate the value of lidocaine in the comprehensive treatment of tinnitus. Methods: the literature of lidocaine in the treatment of tinnitus was selected for strict quality evaluation and data extraction by searching the databases of China knowledge Network, Wanfang, Vipp, Pubmed,SciVerse ScienceDirect,Springer,OVID and so on. Meta software was used to analyze the related indexes of the literature. Results: a total of 16 articles and 1 203 patients were included in the literature, 14 in Chinese and 2 in English. The course of tinnitus was 7h~20 year. The evaluation of curative effect was mainly based on loudness grade, severity scale or subjective feeling. All the literatures did not specifically mention the duration of curative effect, only with the words "short" and "short". Among 133 cases in intravenous injection group, the total effective rate was 73.4% (98 cases), the total effective rate was 74.0% (37 cases) in tympanic injection group (50 cases), and the total effective rate was 87.7% (291 cases) in acupoint injection group (332 cases). The number of cases in each control group was 125, 40 and 326. the total effective rate was 42.4%. The results of 58.3%.Meta analysis and analysis showed that the total effective rate was 42.4%. The total effective rate of lidocaine in the treatment of tinnitus was higher than that of traditional drugs (P 0.05). Conclusion: lidocaine has a certain inhibitory effect on tinnitus, although the effect of lidocaine is short. As an auxiliary measure of comprehensive treatment of tinnitus, it is helpful to shorten the time to adapt to tinnitus rather than eliminate tinnitus, and attention should be paid to the analysis in clinical application.
