【摘要】：目的:比較小切口劈核法與超聲乳化白內(nèi)障摘除術(shù)治療老年性硬核性白內(nèi)障的療效。方法:選擇70例(78眼)老年性硬核性白內(nèi)障患者為研究對(duì)象,采用隨機(jī)數(shù)字表法分為觀察組和對(duì)照組各35例(39眼)。對(duì)照組患者接受超聲乳化白內(nèi)障摘除術(shù),觀察組患者接受小切口劈核法白內(nèi)障摘除術(shù)。比較2組患者的視力水平、散光程度、不良反應(yīng)、生活質(zhì)量。結(jié)果:2組患者術(shù)前視力水平差異無統(tǒng)計(jì)學(xué)意義(P0.05);術(shù)后2組患者視力水平較術(shù)前均顯著改善(P0.01),觀察組患者術(shù)后4周和8周視力水平均高于對(duì)照組(P0.01和P0.05)。術(shù)前2組散光度差異無統(tǒng)計(jì)學(xué)意義(P0.05);2組患者散光度術(shù)后1周均明顯升高,8周后均呈下降趨勢(shì)(P0.01和P0.05);術(shù)后同時(shí)期2組散光度差異均無統(tǒng)計(jì)學(xué)意義(P0.05)。觀察組不良反應(yīng)發(fā)生率為25.64%,低于對(duì)照組的48.72%(P0.05);2組患者術(shù)后的生活質(zhì)量評(píng)分差異無統(tǒng)計(jì)學(xué)意義(P0.05)。結(jié)論:綜合考慮視力水平、散光程度、生活質(zhì)量以及術(shù)后并發(fā)癥情況,小切口劈核法白內(nèi)障摘除術(shù)可作為基層醫(yī)院治療老年性硬核白內(nèi)障的方法。
[Abstract]:Objective: to compare the efficacy of small incision nucleus splicing and phacoemulsification in the treatment of elderly hard nucleus cataract. Methods: 70 patients (78 eyes) with senile hard nucleus cataract were randomly divided into observation group (n = 35) and control group (n = 39). The patients in the control group were treated with phacoemulsification and the patients in the observation group were treated with small incision nucleus split cataract extraction. The visual acuity, astigmatism, adverse reactions and quality of life were compared between the two groups. Results: there was no significant difference in preoperative visual acuity between the two groups (P 0.05). The visual acuity level of the two groups was significantly improved compared with that before operation (P 0.01), and the visual acuity level of the observation group was higher than that of the control group at 4 and 8 weeks after operation (P 0.01 and P 0.05). There was no significant difference in astigmatism between the two groups before operation (P 0.05), but increased significantly in both groups one week after operation, and decreased after 8 weeks (P 0.01 and P 0.05). There was no significant difference in astigmatism between the two groups at the same time after operation (P 0.05). The incidence of adverse reactions in the observation group was 25.64%, which was lower than that in the control group (48.72%, P 0.05), but there was no significant difference in the quality of life score between the two groups (P 0.05). Conclusion: considering the level of visual acuity, the degree of astigmatism, the quality of life and the postoperative complications, small incision nucleus split cataract extraction can be used as a method for the treatment of senile hard nucleus cataract in grass-roots hospitals.
