【摘要】：目的探討神經(jīng)生長(zhǎng)因子(NGF)凝膠對(duì)原發(fā)性閉角型青光眼患者術(shù)后視神經(jīng)的保護(hù)作用。方法選取60例86眼原發(fā)性青光眼患者為研究對(duì)象,隨機(jī)分為觀察組(小梁切除術(shù)后采用NGF凝膠做后續(xù)治療)28例42眼和對(duì)照組(術(shù)后采用三磷酸腺苷二鈉片作后續(xù)治療)32例44眼,所有患者在結(jié)束手術(shù)后堅(jiān)持用藥4 w,比較兩組患者術(shù)后用藥前和術(shù)后2、6個(gè)月的視力、眼壓、視野、視網(wǎng)膜神經(jīng)纖維層厚度(RNFL)和視乳頭杯盤比。結(jié)果觀察組患者術(shù)后用藥前視力為0.41±0.17,術(shù)后2個(gè)月為0.50±0.20,術(shù)后6個(gè)月為0.49±0.25,對(duì)照組患者術(shù)后用藥前視力為0.42±0.22,術(shù)后2個(gè)月為0.42±0.19,術(shù)后6個(gè)月為0.41±0.28,觀察組術(shù)后2、6個(gè)月的視力均高于同期的對(duì)照組(P均0.05)。兩組患者眼壓在術(shù)后用藥前、術(shù)后2、6個(gè)月均沒有統(tǒng)計(jì)學(xué)差異。觀察組視野平均視敏度在術(shù)后2、6個(gè)月均明顯高于對(duì)照組,視野平均缺損值在術(shù)后6個(gè)月低于對(duì)照組(P0.05)。觀察組患者在術(shù)后6個(gè)月時(shí),RNFL明顯大于對(duì)照組,杯盤比明顯小于對(duì)照組(P0.05);但在術(shù)后2個(gè)月兩組患者無統(tǒng)計(jì)學(xué)差異。結(jié)論NGF凝膠用于原發(fā)性青光眼患者術(shù)后治療有顯著的臨床效果,對(duì)視神經(jīng)有很好的保護(hù)作用。
[Abstract]:Objective to investigate the protective effect of nerve growth factor (NGF) gel on the optic nerve of patients with primary angle closure glaucoma after operation. Methods 60 patients (86 eyes) with primary glaucoma were selected as subjects. They were randomly divided into observation group (follow-up treatment with NGF gel after trabeculectomy) and control group (follow-up treatment with adenosine triphosphate disodium tablets). The visual acuity, intraocular pressure (IOP), visual field, retinal nerve fiber layer thickness (RNFL) and papillary cup-disk ratio were compared between the two groups before and 2 and 6 months after operation. Results the visual acuity of the patients in the observation group was 0.41 鹵0.17, 0.50 鹵0.20 at 2 months, 0.49 鹵0.25 at 6 months after operation, 0.42 鹵0.22 before and 0.42 鹵0.19 at 2 months after operation, and 0.42 鹵0.22 in the control group. 6 months after operation, the visual acuity of the observation group was 0.41 鹵0.28, and the visual acuity of the observation group was higher than that of the control group at 2 and 6 months after operation (P 0.05). There was no significant difference in IOP between the two groups before and 2 and 6 months after operation. The visual field average visual acuity in the observation group was significantly higher than that in the control group at 2 months and 6 months after operation, and the mean visual field defect value was lower than that in the control group at 6 months after operation (P0.05). At 6 months after operation, the RNFL of the observation group was significantly higher than that of the control group, and the ratio of cup to plate was significantly lower than that of the control group (P0.05), but there was no statistical difference between the two groups at 2 months after operation. Conclusion NGF gel has significant clinical effect and good protective effect on optic nerve in patients with primary glaucoma after operation.
