【摘要】：目的:應(yīng)用多焦視網(wǎng)膜電流圖(multifocal electroretinogram,mfERG)及光學(xué)相干斷層掃描(optical coherence tomography,OCT)分析青少年孔源性視網(wǎng)膜脫離外路術(shù)前術(shù)后視網(wǎng)膜解剖結(jié)構(gòu)的變化和功能恢復(fù)的關(guān)系。方法:收集2015年6月-2016年6月于我院就診的孔源性視網(wǎng)膜脫離患者38例(39眼),其中男25例(26眼),女13例(13眼),年齡30歲,PVRC級(jí)。所有患者均由同一經(jīng)驗(yàn)豐富的眼底醫(yī)生行外路手術(shù)。術(shù)前以及術(shù)后1m,3m,6m行mfERG及OCT檢查,分別記錄1環(huán)及鼻、顳側(cè)視網(wǎng)膜P1波平均振幅密度和黃斑中心凹及其鼻、顳側(cè)ILM-RPE平均厚度,通過(guò)軟件進(jìn)行統(tǒng)計(jì)分析。結(jié)果:本組患者術(shù)前1d,術(shù)后1m,3m,6m mfERG 1環(huán)及鼻,顳側(cè)視網(wǎng)膜P1波平均振幅密度:兩側(cè)觀察區(qū)P1波平均振幅隨時(shí)間持續(xù)升高,且鼻側(cè)觀察區(qū)平均振幅密度升高明顯,差異有統(tǒng)計(jì)學(xué)意義(P0.05);1環(huán)術(shù)前1d至術(shù)后1m P1波平均振幅密度降低,之后隨時(shí)間逐漸升高,差異有統(tǒng)計(jì)學(xué)意義(P0.05)。患者術(shù)前1d,術(shù)后1m,3m,6m OCT后極部視網(wǎng)膜ILM-RPE平均厚度持續(xù)降低,并維持在正常范圍內(nèi),且黃斑顳側(cè)觀察區(qū)ILM-RPE平均厚度明顯減少,差異有統(tǒng)計(jì)學(xué)意義(P0.05)。結(jié)論:1.青少年孔源性視網(wǎng)膜脫離選擇外路手術(shù)對(duì)于視網(wǎng)膜解剖結(jié)構(gòu)和功能的恢復(fù)有良好效果。2.OCT顯示殘余黃斑視網(wǎng)膜下積液是影響青少年孔源性視網(wǎng)膜脫離外路術(shù)后視網(wǎng)膜功能恢復(fù)的主要原因之一。3.mfERG是客觀評(píng)價(jià)后極部各區(qū)域視網(wǎng)膜功能變化的重要手段。
[Abstract]:Objective: To study the relationship between the changes of the retinal anatomical structure and the function recovery after the pre-operation of the rhegmatogenous retinal detachment in the adolescent with multi-focal retinal current (mfrERG) and optical coherence tomography (OCT). Methods:38 cases (39 eyes) of the rhegmatogenous retinal detachment in our hospital from June 2015 to June 2016 were collected, of which 25 were male (26 eyes),13 (13 eyes),30 years old and PVRC. All patients were operated by the same experienced ophthalmologist. MfERG and OCT were performed before and after 1 m,3 m and 6 m, respectively. The mean amplitude density of the P1 wave and the mean thickness of the central and the nose and the lateral side of the retina were recorded in 1 ring and the nose, and the mean thickness of the lateral ILM-RPE was measured by the software. Results: The average amplitude density of P1-wave in the two-side observation region increased with the time, and the mean amplitude density of the nasal-side observation region was significantly higher than that of the two-side observation region (P0.05). The mean amplitude density of the 1-m-1 wave before and after 1-ring operation was decreased, and then the difference was statistically significant (P0.05). The mean thickness of the retinal ILM-RPE was decreased and the mean thickness of ILM-RPE in the observation area of the side of the macula was significantly reduced and the difference was statistically significant (P0.05). Conclusion:1. 2. OCT showed that residual macular subretinal effusion was one of the main causes of retinal function recovery after retinal detachment. The changes of retinal function in various regions of the department.
