【摘要】：目的研究分析糖尿病視網膜病變進展危險因素。方法回顧性分析2012年7月-2015年7月收治的137例糖尿病患者的臨床資料,按照糖尿病患者的視網膜病變情況分為3組,將43例未發(fā)生視網膜病變的患者作為對照組,46例非增生型視網膜病變作為觀察組,48例增生型視網膜病變作為試驗組。統計糖尿病血壓、血糖、糖化血紅蛋白、血脂、白蛋白肌酐比值等指標情況,并利用多因素非條件logistic回歸分析,觀察糖尿病視網膜病變進展高危因素。結果 3組患者的糖化血紅蛋白、總膽固醇、三酰甘油、低密度脂蛋白膽固醇、高密度脂蛋白膽固醇、體質量指數、餐后2 h血糖及空腹血糖指標差異均無統計學意義(P0.05);病程時間、血管生成素2、血管內皮生長因子及尿白蛋白肌酐比值組間對比,差異均有統計學意義(P0.05);多因素logistic回歸分析結果顯示,糖尿病病程、糖化血紅蛋白、收縮壓、尿白蛋白肌酐比值及血管內皮生長因子均與糖尿病視網膜病變因素獨立相關(P0.05)。結論糖尿病患者病程的增長,血糖、血壓狀態(tài)不穩(wěn)以及血管生成素2、血管內皮生長因子的升高均可造成糖尿病視網膜病變的進展。
[Abstract]:Objective to study the risk factors of progression of diabetic retinopathy. Methods the clinical data of 137 diabetic patients admitted from July 2012 to July 2015 were retrospectively analyzed. According to the retinopathy of diabetic patients, the patients were divided into 3 groups, 43 patients without retinopathy as control group. 46 cases of non-proliferative retinopathy as observation group, 48 cases of proliferative retinopathy as experimental group. The blood pressure, blood glucose, glycosylated hemoglobin, blood lipids and albumin creatinine ratio were analyzed. Multivariate logistic regression analysis was used to observe the risk factors for progression of diabetic retinopathy. Results the glycosylated hemoglobin, total cholesterol, triacylglycerol, low density lipoprotein cholesterol, high density lipoprotein cholesterol, body mass index, 2 h postprandial blood glucose and fasting blood glucose were not significantly different (P0.05). The duration of the disease, angiopoietin 2, vascular endothelial growth factor and urinary albumin creatinine ratio between groups, the differences were statistically significant (P0.05); Multivariate logistic regression analysis showed that diabetes course, glycosylated hemoglobin, systolic blood pressure, urinary albumin creatinine ratio and vascular endothelial growth factor were significantly correlated with diabetic retinopathy (P0.05). Conclusion the progression of diabetic retinopathy can be caused by the increase of course of diabetes, unstable blood glucose, unstable blood pressure, and the increase of angiopoietin 2 and vascular endothelial growth factor.
