【摘要】：目的:采用病例對照研究方法分析單側(cè)先天性小耳畸形外耳道閉鎖患者與聽力正常者在安靜及噪聲環(huán)境中言語識別率(SDS)的差異,了解單側(cè)外耳道閉鎖患者在噪聲下的言語識別結(jié)果,為臨床早期干預(yù)提供科學(xué)依據(jù)。方法:單側(cè)先天性小耳畸形合并外耳道閉鎖患者(畸形組)20例,同時(shí)選取年齡完全匹配的正常受試者20例作為對照組,應(yīng)用普通話言語測聽材料,在聲場下測試所有受試者安靜及噪聲環(huán)境中的SDS。結(jié)果:聲場內(nèi)安靜條件下70dB SPL給聲,畸形組與對照組的SDS比較差異無統(tǒng)計(jì)學(xué)意義。信噪分離時(shí),畸形組(言語信號在患側(cè)、噪聲在健側(cè))與對照組(右側(cè)言語聲刺激,左側(cè)噪聲刺激)比較差異有統(tǒng)計(jì)學(xué)意義(單音節(jié)、雙音節(jié)、語句;S/N=0和S/N=-10)(P0.05);當(dāng)畸形組言語信號刺激健側(cè)、噪聲刺激患側(cè)時(shí),兩組比較差異無統(tǒng)計(jì)學(xué)意義。信噪同側(cè)時(shí),兩組的單音節(jié)詞的SDS差異有統(tǒng)計(jì)學(xué)意義(S/N=0和S/N=-5)(P0.05),而雙音節(jié)詞和語句的SDS差異無統(tǒng)計(jì)學(xué)意義(P0.05)。結(jié)論:單側(cè)先天性小耳畸形外耳道閉鎖患者在噪聲條件下SDS低于正常受試者。對患耳聽力進(jìn)行干預(yù)可提高患者噪聲條件下的SDS。
[Abstract]:Objective: to analyze the difference of speech recognition rate (SDS) between patients with unilateral congenital microauricle atresia and normal hearing in quiet and noisy environment by case-control study. To understand the results of speech recognition in patients with unilateral external auditory canal atresia under noise, and to provide scientific basis for early clinical intervention. Methods: twenty patients with unilateral congenital microtia complicated with atresia of external auditory meatus (deformity group) and 20 normal subjects (age matched) were selected as control group. Testing SDS. in quiet and noisy environments for all subjects in sound field Results: there was no significant difference in SDS between the abnormal group and the control group when 70dB SPL was given under the quiet condition in the sound field. When the signal was separated, there were significant differences between the abnormal group (speech signal in the affected side, the noise in the healthy side) and the control group (right speech sound stimulation, left side noise stimulation) (monosyllabic, disyllabic, sentence). S/N=0 and S/N=-10 (P0.05); when the speech signal stimulates the contralateral side and the noise-stimulating side in the deformity group, there is no significant difference between the two groups. The SDS of monosyllabic words in the two groups was significantly different (S/N=0 and S/N=-5) (P0.05), while the SDS difference of dicyllabic words and sentences had no statistical significance (P0.05). Conclusion: the SDS of patients with unilateral congenital microauricle atresia is lower than that of normal subjects. Intervention of hearing in affected ears can improve SDS. in patients with noise
