【摘要】：目的:對比鼻咽癌殘留的保守治療與鼻內(nèi)鏡下手術(shù)治療的效果與安全性。方法:鼻咽癌殘留患者82例,根據(jù)入院順序分為鼻內(nèi)鏡組與挽救性放療組各41例。挽救性放療組給予放療保守治療,鼻內(nèi)鏡組給予鼻內(nèi)鏡下手術(shù)治療,觀察兩組的預(yù)后療效與并發(fā)癥發(fā)生情況。結(jié)果:治療6個月后鼻內(nèi)鏡組與挽救性放療組的有效率分別為92.7%和75.6%,鼻內(nèi)鏡組的有效率高于挽救性放療組(P0.05)。兩組均未出現(xiàn)鼻咽部大出血及鄰近重要器官損傷等嚴(yán)重并發(fā)癥,其他并發(fā)癥經(jīng)對癥處理后好轉(zhuǎn)。鼻內(nèi)鏡組的總生存時間、局部無復(fù)發(fā)生存時間及遠處無轉(zhuǎn)移生存時間分別為(34.24±2.19)個月、(28.45±1.98)個月和(26.33±2.88)個月,均明顯高于挽救性放療組的(26.46±1.98)個月、(21.87±2.09)個月和(20.44±3.19)個月(P0.05)。結(jié)論:相對于保守放療,鼻內(nèi)鏡下手術(shù)治療鼻咽癌殘留能提高療效,安全性良好,延長了患者的生存時間,有很好的應(yīng)用價值。
[Abstract]:Objective: to compare the efficacy and safety of conservative treatment and endoscopic surgery for nasopharyngeal carcinoma. Methods: 82 patients with residual nasopharyngeal carcinoma were divided into nasal endoscopic group and salvage radiotherapy group according to admission order. The patients in the salvage radiotherapy group were treated with conservative radiotherapy, while the patients in the endoscopic group were treated with endoscopic surgery. The prognosis and complications of the two groups were observed. Results: after 6 months of treatment, the effective rates of endoscopic group and salvage radiotherapy group were 92.7% and 75.6%, respectively. The effective rate of nasal endoscopy group was higher than that of salvage radiotherapy group (P0.05). There were no serious complications such as nasopharyngeal hemorrhage and adjacent important organ injury in both groups, and other complications were improved after symptomatic treatment. The total survival time, local non-recurrence survival time and distant non-metastasis survival time of endoscopic group were (34.24 鹵2.19) months, (28.45 鹵1.98) months and (26.33 鹵2.88) months, respectively. They were significantly higher than those in the rescue radiotherapy group (26.46 鹵1.98 months, 21.87 鹵2.09 months and 20.44 鹵3.19 months, P0.05). Conclusion: compared with conservative radiotherapy, endoscopic surgery for residual nasopharyngeal carcinoma can improve the efficacy, safety and prolong the survival time of patients.
