【摘要】：目的:探討糖皮質(zhì)激素聯(lián)合內(nèi)鏡治療對(duì)鼻息肉患者血清炎癥因子的影響及近期療效。方法:94例鼻息肉患者,分為觀察組(鼻內(nèi)鏡手術(shù)聯(lián)合糖皮質(zhì)激素治療,n=45)和對(duì)照組(鼻內(nèi)鏡手術(shù)治療,n=49),于手術(shù)前和手術(shù)后1周時(shí),抽取兩組患者清晨靜脈血,采用酶聯(lián)免疫吸附試驗(yàn)法(ELISA)測(cè)定血清中白細(xì)胞介素2(IL-2)、IL-5及IL-17的水平,比較兩組患者術(shù)后臨床效果及1年復(fù)發(fā)率。結(jié)果:治療后,觀察組患者總有效率91.11%高于對(duì)照組;63.27%(χ~2=10.15,P=0.002);治療前,兩組患者血清IL-2、IL-5及IL-17水平比較,差異無(wú)統(tǒng)計(jì)學(xué)意義(P0.05);治療后,兩組患者血清IL-5及IL-17水平低于治療前(P0.05);治療后,觀察組患者血清IL-5、IL-17水平明顯低于對(duì)照組(P0.05);觀察組患者術(shù)后隨訪1年鼻息肉復(fù)發(fā)率明顯低于對(duì)照組(χ~2=4.467,P=0.031)。結(jié)論:內(nèi)鏡聯(lián)合糖皮質(zhì)激素治療鼻息肉可以有效減輕患者炎癥反應(yīng),降低復(fù)發(fā)率。
[Abstract]:Objective: to investigate the effect of glucocorticoid combined with endoscopic therapy on serum inflammatory factors in nasal polyps. Methods: 94 patients with nasal polyps were divided into two groups: the observation group (nasal endoscopic surgery combined with glucocorticoid therapy, n = 45) and the control group (nasal endoscopic surgery, n = 49). The serum levels of interleukin 2 (IL-2), IL-5 and IL-17 were measured by (ELISA). The clinical effect and recurrence rate of one year were compared between the two groups. Results: after treatment, the total effective rate of patients in the observation group was 91.11% higher than that in the control group, 63.27% (蠂 ~ 2 + 10.15 P < 0.002). Before treatment, there was no significant difference in serum IL-2,IL-5 and IL-17 levels between the two groups (P0.05); after treatment, the levels of serum IL-5 and IL-17 in the two groups were lower than those before treatment (P0.05). After treatment, the serum IL-5,IL-17 level in the observation group was significantly lower than that in the control group (P0.05), and the recurrence rate of nasal polyps in the observation group was significantly lower than that in the control group after 1 year follow-up (蠂 ~ (2) 4.467). Conclusion: endoscopy combined with glucocorticoid can effectively reduce inflammatory reaction and relapse rate of nasal polyps.
