【摘要】：目的:研究Ultra Q-YAG玻璃體消融術(shù)治療不同年齡段臨床有癥狀飛蚊癥患者的療效及安全性。方法:回顧性隊(duì)列研究。對(duì)2014-09/2015-05在北京愛(ài)爾英智眼科醫(yī)院臨床確診為玻璃體混濁的患者263例340眼納入研究,所有患者均行視力、眼壓、裂隙燈、散瞳查眼底、B超檢查,并記錄玻璃體混濁物的形態(tài),排除眼底病變。根據(jù)患者年齡分為兩組:A組(30歲)78例82眼,玻璃體混濁的形態(tài)多為細(xì)點(diǎn)、絲狀、網(wǎng)狀;B組(45歲)185例258眼,玻璃體混濁的形態(tài)多為Weiss環(huán),半透明絮狀或致密纖維膜狀。均排除外傷與眼內(nèi)病變所致混濁,30~45歲患者由于混濁因素不典型被剔除。所有患者均由同一有經(jīng)驗(yàn)醫(yī)師行YAG玻璃體消融術(shù)(Ellex Medical Lasers),對(duì)比分析治療后1mo飛蚊癥狀緩解情況。結(jié)果:行問(wèn)卷調(diào)查,根據(jù)患者問(wèn)卷得分將術(shù)后癥狀改善程度分為基本無(wú)改善(1~2分)、部分改善(3~5分)、顯著改善(6~10分)。1mo后A組無(wú)改善9眼(11.0%),部分改善57眼(69.5%),顯著改善16眼(19.5%);B組無(wú)改善0眼,部分改善23眼(8.9%),顯著改善235眼(91.1%);所有患者無(wú)1例并發(fā)癥發(fā)生。結(jié)論:YAG玻璃體消融術(shù)治療飛蚊癥安全有效。對(duì)于年齡30歲年齡組的飛蚊癥患者,有改善但顯著改善的幾率不高。而對(duì)于年齡45歲年齡組,因玻璃體后脫離導(dǎo)致的玻璃體混濁,患者效果更佳。
[Abstract]:Objective: to study the efficacy and safety of Ultra Q-YAG vitreous ablation in the treatment of symptomatic mosquitoes in different age groups. Methods: retrospective cohort study. In the study, 263 cases (340 eyes) with vitreous opacity diagnosed clinically in Beijing Elin Eye Hospital on May 05, 2014-09 / 2015-05, were included in the study. All patients underwent visual acuity, intraocular pressure, slit lamp, mydriasis, fundus examination and B-ultrasound examination. The morphology of vitreous opacity was recorded and the fundus lesion was excluded. According to the age of the patients, they were divided into two groups: group A (30 years old), 78 cases (82 eyes), the shape of vitreous opacity was mostly fine, filamentous and reticular; B group (45 years old) 185 cases 258 eyes, the vitreous opacity shape is mostly Weiss ring, translucent flocculation or dense fibrous membrane shape. Opacity caused by trauma and intraocular lesion was excluded, and 30 ~ 45 years old patients were excluded because of atypical opacity factors. All the patients were treated with YAG vitreous ablation by the same experienced physician. (Ellex Medical Lasers), was compared to analyze the remission of 1mo after treatment. Results: according to the scores of the patients, the degree of symptom improvement was divided into three groups: no improvement (1 ~ 2 points), partial improvement (3 ~ 5 points), significant improvement (6 ~ 10 points), and no improvement in 9 eyes (11.0%) in group A after 1mo. Partial improvement was observed in 57 eyes (69.5%) and significant improvement in 16 eyes (19.5%). In group B, there was no improvement in 0 eyes, partial improvement in 23 eyes (8.9%), significant improvement in 235 eyes (91.1%), and no complication occurred in all patients. Conclusion: YAG vitreous ablation is safe and effective in the treatment of mosquito disease. For the 30-year-old group, the chances of improvement but significant improvement were low. For the 45-year-old group, the effect of vitreous opacity due to posterior vitreous detachment was better.
