【摘要】：目的:分析良性陣發(fā)性位置性眩暈(BPPV)變位試驗(yàn)假陰性的比例和原因。方法:回顧性分析2009-02-2009-04期間就診于北京大學(xué)人民醫(yī)院耳鼻咽喉科門診具有BPPV典型臨床癥狀、變位試驗(yàn)確診、經(jīng)過復(fù)位治療痊愈的68例BPPV患者的資料。所有患者均行變位試驗(yàn)檢查,首次變位試驗(yàn)誘發(fā)出典型眼震者為陽(yáng)性組;首次檢查陰性的患者,搖頭后或隔日復(fù)診再次變位試驗(yàn)檢查均獲得典型眼震為假陰性組,經(jīng)過復(fù)位治療癥狀痊愈。結(jié)果:13例(19.12%)患者首次變位試驗(yàn)出現(xiàn)陰性,經(jīng)過反復(fù)變位試驗(yàn),出現(xiàn)陽(yáng)性,其中11例在檢查當(dāng)天搖頭后再次變位試驗(yàn)出現(xiàn)典型眼震者,2例隔日復(fù)診變位試驗(yàn)出現(xiàn)典型眼震。假陰性組年齡、性別、病程、半規(guī)管類型與陽(yáng)性組差異無統(tǒng)計(jì)學(xué)意義(P0.05)。結(jié)論:對(duì)臨床疑為BPPV但變位試驗(yàn)陰性的患者,建議搖頭后再次檢查并隨訪復(fù)查以獲得典型眼震明確診斷。
[Abstract]:Objective: to analyze the false negative rate and causes of (BPPV) transposition test in benign paroxysmal positional vertigo. Methods: the clinical data of 68 BPPV patients who were diagnosed by transposition test and were diagnosed by transposition test during 2009-02-2009-04 were analyzed retrospectively in the department of otolaryngology and otolaryngology of people's Hospital of Peking University. All patients were examined by displacement test, the first transposition test induced typical nystagmus as positive group, and the patients who were negative for the first time were found to be false negative group after shaking their head or the next day. The symptoms were cured after reduction. Results: 13 cases (19.12%) were negative in the first transposition test. After repeated translocation test, 11 cases had typical nystagmus after shaking their head on the same day, and 2 cases had typical nystagmus in the second transposition test every other day. There was no significant difference in age, sex, course, semicircular canal type between false negative group and positive group (P0.05). Conclusion: it is suggested that the patients suspected to be BPPV but negative for transposition test should be examined again and followed up to obtain a definite diagnosis of typical nystagmus.
