【摘要】：目的探討兒童慢性鼻-鼻竇炎發(fā)生的影響因素及觀察抗過敏治療慢性鼻-鼻竇炎兒童的治療效果。方法選擇2013年1月至2015年6月收治的93例慢性鼻-鼻竇炎患者,年齡5~18歲。了解患者家族過敏史、以往過敏史和與過敏反應(yīng)相關(guān)的疾病或癥狀,所有患者進(jìn)行身體檢查和皮膚過敏性試驗(yàn)。皮試陽性患者接受抗菌藥物治療,抗組胺藥物治療(抗過敏藥物),局部糖皮質(zhì)激素治療,全身性糖皮質(zhì)激素治療及手術(shù)治療后對患者進(jìn)行隨訪,評估患者治療效果。結(jié)果患者平均年齡(8.3±6.7)歲,男48例(51.6%),女45例(48.4%)。皮膚過敏性試驗(yàn)結(jié)果顯示,陽性患者73例(78.5%),其中男38例(52.1%),女35例(47.9%)。陽性患者抗過敏治療和局部糖皮質(zhì)激素資料顯著改善患者治療效果,其中抗過敏治療患者治愈率為97.7%,局部皮質(zhì)激素治療和功能性鼻內(nèi)鏡鼻竇術(shù)患者治愈率為64.7%和72.3%,抗菌藥物治療不能顯著改善患者治療效果;皮試陽性患者數(shù)量與年齡呈負(fù)相關(guān)。結(jié)論慢性鼻-鼻竇炎患者過敏反應(yīng)或過敏相關(guān)癥狀的發(fā)生率較高,過敏反應(yīng)與慢性鼻-鼻竇炎的發(fā)生密切相關(guān);抗過敏治療和局部糖皮質(zhì)激素治療能改善慢性鼻-鼻竇炎患者的臨床癥狀,抗過敏治療效果優(yōu)于局部糖皮質(zhì)激素治療。
[Abstract]:Objective to investigate the influencing factors of chronic rhinosinusitis in children and to observe the therapeutic effect of anti-allergic therapy on children with chronic rhinosinusitis. Methods 93 patients with chronic rhinosinusitis, aged 5 to 18 years, were selected from January 2013 to June 2015. Understand the patient's family history of allergies, past allergies, and allergy related diseases or symptoms. All patients underwent physical examination and skin allergy tests. Patients with positive skin test received antimicrobial therapy, antihistamine therapy, local glucocorticoid therapy, systemic glucocorticoid therapy and surgical treatment. Results the average age of the patients was (8.3 鹵6.7) years, 48 males (51.6%) and 45 females (48.4%). The results of skin anaphylactic test showed that 73 cases (78.5%) were positive, 38 cases (52.1%) were males and 35 cases (47.9%) were females. The antiallergic therapy and local glucocorticoid data in positive patients significantly improved the therapeutic effect. The cure rate of the patients treated with antiallergic therapy was 97.7.The cure rate of local corticosteroid therapy and functional nasal endoscopic sinus surgery was 64.7% and 72.3% respectively. Antimicrobial therapy could not significantly improve the curative effect of the patients, and the number of patients with positive skin test was negatively correlated with age. Conclusion the incidence of allergic reaction or allergy related symptoms in patients with chronic rhinosinusitis is high, and the allergic reaction is closely related to the occurrence of chronic rhinosinusitis. Antiallergic therapy and local glucocorticoid therapy can improve the clinical symptoms of patients with chronic rhinosinusitis and the antiallergic effect is better than that of local glucocorticoid therapy.
