【摘要】：目的：探討伴有白內(nèi)障的玻璃體視網(wǎng)膜疾病患者行小切口白內(nèi)障囊外摘除聯(lián)合玻璃體切除及人工晶體植入術(shù)的臨床療效。 方法：回顧分析伴有白內(nèi)障的玻璃體視網(wǎng)膜疾病患者49例（51眼）進(jìn)行小切口白內(nèi)障囊外摘除聯(lián)合玻璃體切除術(shù)，，21眼同時(shí)行Ⅰ期人工晶體植入術(shù)，觀察術(shù)后視力改善程度、術(shù)中術(shù)后并發(fā)癥及處理的情況。 結(jié)果：術(shù)后51眼中視力提高30眼（58.82%），術(shù)中Ⅰ期植入人工晶體的21眼中，視力提高14眼（66.67%）。術(shù)中主要并發(fā)癥包括角膜上皮水腫1眼（1.96%）；醫(yī)源性裂孔2眼（3.92%）；眼底出血1眼（1.96%）；后囊膜破裂1眼（1.96%）。術(shù)后主要并發(fā)癥包括角膜水腫24眼（47.06%），前房閃輝18眼（35.29%），瞳孔區(qū)滲出膜4眼（7.84%），瞳孔區(qū)虹膜后粘連4眼（7.84%），一過(guò)性眼壓升高7眼（13.73%），前房積血5眼（9.80%），繼發(fā)性青光眼3眼（5.88%）等。 結(jié)論：小切口白內(nèi)障囊外摘除、玻璃體切除聯(lián)合人工晶體植入術(shù)治療伴有白內(nèi)障的玻璃體視網(wǎng)膜疾病是安全有效的，影響術(shù)后視力改善的主要因素為眼底視網(wǎng)膜病變和眼外傷的嚴(yán)重程度，手術(shù)成功的關(guān)鍵是選擇合適的病例和手術(shù)醫(yī)生嫻熟的技術(shù)。
[Abstract]:Objective: to investigate the clinical effect of small incision extracapsular cataract extraction combined with vitrectomy and intraocular lens implantation in patients with vitreoretinal diseases with cataract. Methods: 49 cases (51 eyes) of vitreoretinal diseases with cataract were treated with extracapsular cataract extraction and vitrectomy through small incision. Complications during and after operation and management. Results: the visual acuity was improved in 30 eyes (58.82%) in 51 eyes and 14 eyes (66.67%) in 21 eyes with intraocular lens implantation. The main complications included corneal epithelial edema in 1 eye (1.96%), iatrogenic hole in 2 eyes (3.92%), fundus hemorrhage in 1 eye (1.96%) and posterior capsule rupture in 1 eye (1.96%). The main postoperative complications included corneal edema in 24 eyes (47.06%), anterior atrial flash glow in 18 eyes (35.29%), pupillary effusion in 4 eyes (7.84%), posterior synechia in 4 eyes (7.84%), transient intraocular hypertension in 7 eyes (13.73%), hyphema in 5 eyes (9.80%), secondary glaucoma in 3 eyes (5.88%). Conclusion: small incision extracapsular cataract extraction vitrectomy combined with intraocular lens implantation is safe and effective in the treatment of vitreoretinal diseases with cataract. The main factors affecting the improvement of visual acuity after operation were the severity of retinopathy and ocular trauma. The key to success of the operation was the selection of appropriate cases and the skillful technique of the surgeon.
