【摘要】：目的：經(jīng)睫狀體平坦部硅油取出聯(lián)合白內(nèi)障超聲乳化手術(shù)按照取硅油和摘除白內(nèi)障的先后順序可分為兩種術(shù)式,探討其各自特點(diǎn)、手術(shù)的適應(yīng)癥、以及臨床操作技巧。 方法：經(jīng)睫狀體平坦部鞏膜切口取出硅油聯(lián)合白內(nèi)障超聲乳化摘除及人工晶狀體植入術(shù)�；仡櫺苑治隽藘山M共40例(40只眼)硅油填充眼行聯(lián)合手術(shù)治療。A組20例,先摘除白內(nèi)障后取出硅油；B組20例,先取出硅油后摘除白內(nèi)障；術(shù)后根據(jù)門(mén)診病歷資料進(jìn)行隨訪觀察6個(gè)月。用卡方檢驗(yàn)分析比較兩組手術(shù)方式術(shù)中及術(shù)后并發(fā)癥。 結(jié)果：兩組聯(lián)合手術(shù)均順利完成,最佳矯正視力0.05—0.1者13例,0.1-0.5者9例；術(shù)后因復(fù)發(fā)性視網(wǎng)膜脫離需再次手術(shù)治療者2例；術(shù)后并發(fā)癥主要有硅油殘留、角膜水腫、黃斑囊樣水腫、復(fù)發(fā)性視網(wǎng)膜脫離及晶狀體皮質(zhì)殘留等。 結(jié)論：經(jīng)睫狀體平坦部取出硅油聯(lián)合白內(nèi)障超聲乳化手術(shù)按照取硅油和摘除白內(nèi)障的先后順序可分為兩種術(shù)式,這兩種術(shù)式都是安全、可行的,但在手術(shù)過(guò)程中又有其各自特點(diǎn),嚴(yán)格把握手術(shù)的適應(yīng)癥,手術(shù)過(guò)程中采取必要的對(duì)癥措施以及具備相當(dāng)?shù)氖中g(shù)技巧是手術(shù)能否成功的關(guān)鍵。
[Abstract]:Objective: to study the characteristics, indications and clinical techniques of phacoemulsification combined with silicone oil extraction from the flat part of the ciliary body. Methods: silicone oil was removed from the scleral incision of the flat ciliary body combined with phacoemulsification and intraocular lens implantation. A retrospective analysis of 40 cases (40 eyes) of silicone oil filled eyes was performed in two groups: group A (n = 20), group B (n = 20). The patients were followed up for 6 months according to the data of outpatient medical records. The intraoperative and postoperative complications of the two groups were analyzed by chi-square test. Results: the combined operation was completed successfully in both groups. The best corrected visual acuity was 0.05-0.1 in 13 cases and 0.1-0.5 in 9 cases. Macular cystic edema, recurrent retinal detachment and residual lens cortex. Conclusion: the silicone oil removed from the flat part of the ciliary body and phacoemulsification can be divided into two types according to the sequence of silicone oil extraction and cataract extraction, both of which are safe and feasible. However, there are their own characteristics in the process of operation. The key to the success of the operation is to strictly grasp the indication of the operation, to take necessary measures to deal with the disease in the course of the operation and to have appropriate surgical skills.
