【摘要】：目的應(yīng)用共聚焦顯微鏡觀察干眼癥患者的角膜形態(tài)改變。方法運(yùn)用日本Nidek公司共聚焦顯微鏡(Confoscan 4.0)對(duì)正常人群和干眼癥患者各50例(100只眼)的中央?yún)^(qū)角膜上皮細(xì)胞的密度和異型性、上皮下神經(jīng)纖維形態(tài)、朗格漢斯細(xì)胞的浸潤(rùn)和內(nèi)皮細(xì)胞密度進(jìn)行觀察,并對(duì)結(jié)果進(jìn)行統(tǒng)計(jì)學(xué)分析。結(jié)果共聚焦顯微鏡觀察下,干眼癥患者和正常人群的角膜中央?yún)^(qū)表層鱗狀上皮細(xì)胞、翼狀上皮細(xì)胞、上皮基底層細(xì)胞的細(xì)胞密度比較差異有統(tǒng)計(jì)學(xué)意義(P0.05)。干眼癥患者的中央?yún)^(qū)角膜上皮細(xì)胞異型性增多(t=14.3,P0.01),少量細(xì)胞呈激活狀態(tài),上皮下神經(jīng)纖維明顯增粗,走行紊亂,伴朗格漢斯細(xì)胞浸潤(rùn),內(nèi)皮細(xì)胞計(jì)數(shù)和形態(tài)差異無(wú)統(tǒng)計(jì)學(xué)意義。結(jié)論共聚焦顯微鏡可以從細(xì)胞層面實(shí)時(shí)、無(wú)創(chuàng)觀測(cè)干眼癥患者的角膜形態(tài)的改變,根據(jù)角膜上皮細(xì)胞細(xì)胞密度和異型性、上皮下神經(jīng)纖維形態(tài)、朗格漢斯浸潤(rùn)等病理變化直觀識(shí)別和診斷干眼癥。
[Abstract]:Objective to observe the corneal morphology in patients with xerophthalmia by confocal microscope. Methods Nidek confocal microscope (Confoscan 4.0) was used to study the density and heterogeneity of corneal epithelial cells in the central area of 50 normal subjects and 50 dry eye patients (100 eyes), and the morphology of epigastric nerve fibers. The infiltration of Langerhans cells and the density of endothelial cells were observed and the results were analyzed statistically. Results under confocal microscope, there were significant differences in cell density between dry eye patients and normal subjects in the surface squamous epithelium, airfoil epithelium and basal layer cells of cornea (P0.05). In dry eye patients, the corneal epithelial cells in the central area were increased (t: 14. 3 / P0.01), a small number of cells were activated, and the nerve fibers of the upper subcutaneous were thickened and disordered, accompanied by Langerhans' cell infiltration, and there was no significant difference in the number and morphology of endothelial cells. Conclusion Confocal microscope can be used to observe the changes of corneal morphology in patients with xerophthalmia at the cell level in real time. According to the density and heterogeneity of corneal epithelial cells, the morphology of epigastric nerve fibers can be observed. Langerhans infiltration and other pathological changes visual recognition and diagnosis of dry eye.
