【摘要】：目的觀察長期小劑量克拉霉素治療慢性鼻-鼻竇炎的療效。方法將93例慢性鼻-鼻竇炎(不伴鼻息肉)患者,隨機分為兩組,治療組46例,對照組47例,治療組采用克拉霉素250mg口服,1次/d,聯(lián)合鼻用糖皮質(zhì)激素、黏液促排劑及生理性海水鼻腔盥洗。對照組則單純采用鼻用糖皮質(zhì)激素、黏液促排劑及生理性海水鼻腔盥洗。治療前后采用視覺模擬量表評分(visual analogue scale,VAS)、鼻竇CT評分(Lund Mackay)及臨床療效進行療效評價。結(jié)果兩組治療3個月后的VAS評分、鼻竇CT評分和臨床療效評定均降低,差異有統(tǒng)計學意義(P0.05)。治療組較對照組降低更明顯,差異有統(tǒng)計學意義(P0.05)。且在整個治療期間未發(fā)現(xiàn)患者有任何嚴重的不良反應。結(jié)論長期小劑量克拉霉素聯(lián)合鼻用糖皮激素、黏液促排劑及生理性海水鼻腔盥洗治療慢性鼻-鼻竇炎臨床療效好。
[Abstract]:Objective to observe the effect of long-term low-dose clarithromycin on chronic rhinosinusitis. Methods 93 patients with chronic rhinosinusitis (without nasal polyps) were randomly divided into two groups: treatment group (n = 46) and control group (n = 47). The treatment group was treated with clarithromycin 250mg once a day, combined with glucocorticoid. Mucus draining agent and physiologic sea water nasal lavatory. The control group was treated with nasal glucocorticoid, mucus discharges and physiologic sea water nasal lavatory. Visual analogue scale (visual analogue scale,VAS), paranasal sinus CT score (Lund Mackay) and clinical efficacy were used before and after treatment. Results after 3 months of treatment, the VAS score, sinus CT score and clinical efficacy of the two groups were significantly lower than those of the control group (P0.05). The treatment group was significantly lower than the control group, the difference was statistically significant (P0.05). No serious adverse reactions were found throughout the treatment. Conclusion Long-term low dose clarithromycin combined with glucocorticoid mucus and physiologic seawater nasal lavage is effective in the treatment of chronic rhinosinusitis.
