【摘要】：目的對(duì)原發(fā)性閉角型青光眼合并白內(nèi)障手術(shù)方式進(jìn)行循證醫(yī)學(xué)系統(tǒng)評(píng)價(jià),為手術(shù)方式選擇提供循證醫(yī)學(xué)證據(jù)。方法按照循證醫(yī)學(xué)系統(tǒng)評(píng)價(jià)要求,檢索PubMed、EMBASE、Cochrane Library及CNKI和CBM發(fā)表的相關(guān)中、英文文獻(xiàn)(2005.01~2011.07)。觀察指標(biāo)包括治療前后視力、眼壓、眼壓控制成功率、視野損害進(jìn)展、RNFL、房角粘連變化、并發(fā)癥等。結(jié)果共10篇英文和13篇中文文獻(xiàn)入選。其中Ⅰ~Ⅱ級(jí)證據(jù)11篇,Ⅲ級(jí)證據(jù)7篇,Ⅳ級(jí)證據(jù)5篇,涉及手術(shù)方式包括:LPI、SPI、ECCE、Phaco、Phaco-GSL、Phaco+SPI、PhacoTrab+IOL及小梁切除術(shù)。結(jié)論循證醫(yī)學(xué)系統(tǒng)評(píng)價(jià)證實(shí)超乳手術(shù)可作為PACG合并白內(nèi)障手術(shù)方式。手術(shù)適應(yīng)證方面臨床需要進(jìn)一步大樣本、多中心RCT研究,建議研究中納入有關(guān)視神經(jīng)損害及長(zhǎng)期降眼壓效果的觀察指標(biāo)。
[Abstract]:Objective to evaluate the Evidence-Based Medicine (EBM) system for primary angle-closure glaucoma with cataract. Methods according to the requirements of Evidence-based Medicine (EBM) system evaluation, we searched the Chinese and English literature published by PubMed,EMBASE,Cochrane Library, CNKI and CBM (2005.01 / 2011.07). The visual acuity, the success rate of intraocular pressure control, the progression of visual field damage and RNFL, the changes of atrial angle adhesion, complications and so on were observed before and after treatment.
