【摘要】：目的評價(jià)超聲乳化白內(nèi)障吸除術(shù)對糖尿病性白內(nèi)障術(shù)后角膜內(nèi)皮及厚度等指標(biāo)的影響。方法96例(96眼)2型糖尿病性白內(nèi)障患者(糖尿病組)以及252例(252眼)老年性白內(nèi)障患者(老年組)接受超聲乳化白內(nèi)障吸出聯(lián)合后房型人工晶狀體植入術(shù),比較兩組患者術(shù)前及術(shù)后1周、4周、8周淚膜破裂時(shí)間、基礎(chǔ)淚液分泌、角膜內(nèi)皮細(xì)胞密度以及中央角膜厚度。結(jié)果術(shù)后1周、4周、8周糖尿病組患者的淚膜破裂時(shí)間、基礎(chǔ)淚液分泌、角膜內(nèi)皮細(xì)胞密度以及中央角膜厚度與術(shù)前差異均有統(tǒng)計(jì)學(xué)意義(均為P0.05);老年組患者的角膜內(nèi)皮細(xì)胞密度差異均有統(tǒng)計(jì)學(xué)意義(均為P0.05),而淚膜破裂時(shí)間、基礎(chǔ)淚液分泌以及中央角膜厚度差異均無統(tǒng)計(jì)學(xué)意義(均為P0.05)。與老年組相比,糖尿病組患者術(shù)前及術(shù)后1周、4周、8周淚膜破裂時(shí)間、基礎(chǔ)淚液分泌、角膜內(nèi)皮細(xì)胞密度以及中央角膜厚度差異均有統(tǒng)計(jì)學(xué)意義(均為P0.05)。結(jié)論糖尿病患者淚膜穩(wěn)定性、基礎(chǔ)淚液分泌以及角膜內(nèi)皮細(xì)胞密度均顯著降低,白內(nèi)障超聲乳化吸出術(shù)后這些眼部體征更加顯著,術(shù)中及術(shù)后加強(qiáng)保護(hù)角膜內(nèi)皮及眼表面組織,控制術(shù)后組織反應(yīng)是減少糖尿病性白內(nèi)障術(shù)后并發(fā)癥的重要手段。
[Abstract]:Objective to evaluate the effect of phacoemulsification on corneal endothelium and corneal thickness after diabetic cataract surgery. Methods 96 cases (96 eyes) of type 2 diabetic cataract and 252 cases (252 eyes) of senile cataract were treated with phacoemulsification combined with posterior chamber intraocular lens implantation. The time of lacrimal film rupture, basal tear secretion, corneal endothelial cell density and central corneal thickness were compared between the two groups before and 1 week after operation. Results the lacrimal film rupture time and basic tear secretion were observed in patients with diabetes mellitus at 1 week and 4 weeks and 8 weeks after operation. The corneal endothelial cell density and central corneal thickness were significantly different from those before operation (P0.05), the corneal endothelial cell density of the elderly patients were significantly different (P0.05), and the tear film rupture time was significant (P0.05). There was no significant difference in basal tear secretion and central corneal thickness (P0.05). Compared with the elderly group, there were significant differences in tear film rupture time, basal tear secretion, corneal endothelial cell density and central corneal thickness between the diabetic group and the diabetic group before and 1 week after operation (P0.05). Conclusion the stability of lacrimal film, basal tear secretion and corneal endothelial cell density were significantly decreased in diabetic patients. These ocular signs were more significant after phacoemulsification. During and after cataract phacoemulsification, the corneal endothelium and ocular surface tissue should be protected. Controlling postoperative tissue reaction is an important method to reduce postoperative complications of diabetic cataract.
