【摘要】：目的:采用免疫組織化學實驗并擴大標本量與臨床療效相結合觀察垂體腺苷酸環(huán)化酶激活肽(PACAP)蛋白在慢性鼻-鼻竇炎、鼻息肉、難治性慢性鼻-鼻竇炎及正常鼻黏膜組織中的表達情況,了解PACAP蛋白與慢性鼻-鼻竇炎和鼻息肉發(fā)病、轉歸及預后的關系。方法:采用免疫組織化學SP法分別檢測53例鼻息肉、70例慢性鼻-鼻竇炎、28例難治性慢性鼻-鼻竇炎病變組織和20例正常鼻黏膜組織石蠟標本的PACAP蛋白表達情況。結果:1PACAP蛋白表達于鼻黏膜上皮、腺上皮和杯狀細胞的胞質和胞核中;2鼻息肉、慢性鼻-鼻竇炎、難治性慢性鼻-鼻竇炎和正常鼻黏膜組織之間PACAP蛋白陽性強度構成比差異有統(tǒng)計學意義(χ2=140.902,P0.01)。鼻息肉組織的陽性強度以"+"為主,慢性鼻-鼻竇炎組織的陽性強度以"+++"為主,難治性慢性鼻-鼻竇炎組織的陽性強度以"-~+"為主,正常鼻黏膜組織的陽性強度以"++"為主。結論:PACAP蛋白是機體保護性蛋白,當鼻黏膜受病原體侵襲或理化因素刺激引起炎癥反應時,鼻黏膜上皮、腺上皮和杯狀細胞,尤其是腺體反應性增生明顯,它們產生和分泌PACAP蛋白等抗炎物質增多從而起到炎癥防御作用。當PACAP減少后,對鼻黏膜的抗炎及免疫調節(jié)作用下降,因此炎癥易遷延不愈或致息肉復發(fā)。
[Abstract]:Objective: to observe the expression of pituitary adenylate cyclase activating peptide (PACAP) protein in chronic rhinosinusitis and nasal polyps by immunohistochemistry and clinical efficacy. To investigate the expression of PACAP protein in refractory chronic rhinosinusitis and normal nasal mucosa, and to investigate the relationship between PACAP protein and the pathogenesis, outcome and prognosis of chronic rhinosinusitis and nasal polyps. Methods: immunohistochemical SP method was used to detect the expression of PACAP protein in 53 cases of nasal polyps 70 cases of chronic rhinosinusitis and 28 cases of refractory chronic rhinosinusitis and 20 cases of normal nasal mucosa. Results the protein was expressed in the cytoplasm and nucleus of nasal mucosa epithelium, glandular epithelium and goblet cells. There was a significant difference in the ratio of PACAP positive intensity between refractory chronic rhinosinusitis and normal nasal mucosa (蠂 2 / 140.902 / P0.01). The positive intensity of nasal polyps, chronic rhinosinusitis and refractory chronic rhinosinusitis was mainly "-~". The positive intensity of normal nasal mucosa was mainly ". ConclusionThe fraction PACAP protein is a protective protein. When nasal mucosa is affected by pathogen or stimulated by physical and chemical factors, the nasal mucosa epithelium, glandular epithelium and goblet cells, especially gland reactive hyperplasia, are obvious. They produce and secrete more anti-inflammatory substances, such as PACAP proteins, which play an inflammatory defense role. When PACAP is reduced, the anti-inflammatory and immunomodulatory effects on nasal mucosa are decreased, so inflammation is prone to unhealing or recurrence of polyps.
