眼表綜合分析儀在評(píng)價(jià)干燥綜合征干眼中的臨床意義

【摘要】：目的探討干燥綜合征患者主觀癥狀上的Mc Monnie問卷評(píng)分、眼科檢查和免疫學(xué)相關(guān)檢查等輔助檢查在評(píng)價(jià)干燥綜合征干眼中的臨床意義。方法干燥綜合征干眼患者共102例204眼,其中83人有效完成Mc Monnie問卷調(diào)查以評(píng)估主觀癥狀;101人完成眼表綜合分析儀等眼科相關(guān)檢查,94人完成免疫球蛋白Ig G、Ig A、Ig M、補(bǔ)體C3、補(bǔ)體C4和C反應(yīng)蛋白的檢查,78人完成抗核抗體、抗α胞襯蛋白抗體、抗ENA抗體-SSA和抗ENA抗體-SSB的檢查。采用t檢驗(yàn)和卡方檢驗(yàn)比較相關(guān)數(shù)據(jù)之間的聯(lián)系。結(jié)果主觀問卷評(píng)分與角膜熒光素染色結(jié)果(P=0.019)、脂質(zhì)層顏色(P=0.027)有相關(guān)性。年齡與瞼板腺數(shù)量有相關(guān)性(P=0.022)。雙眼脂質(zhì)層顏色(P=0.015)、角膜熒光素染色(P=0.025)和淚河高度(P=0.039)均與血清中Ig G水平存在相關(guān)性。血清中Ig A水平和BUT結(jié)果有相關(guān)性(P=0.022)。結(jié)論淚膜脂質(zhì)層、瞼板腺、角膜的結(jié)構(gòu)和功能的狀態(tài)與患者的眼部癥狀關(guān)系密切,血清中免疫球蛋白水平與其眼科客觀檢查的指標(biāo)有明顯關(guān)聯(lián),可作為干燥綜合征干眼的整體病情和淚膜角膜損傷程度的重要評(píng)估手段。
[Abstract]:Objective to investigate the clinical significance of Mc Monnie questionnaire, ophthalmology and immunological examination in the evaluation of dry eyes of Sjogren's syndrome (Sjogren's syndrome). Methods 102 cases (204 eyes) of dry eye with Sjogren's syndrome, Among them, 83 patients completed the Mc Monnie questionnaire to evaluate subjective symptoms. 101 patients completed ophthalmological examination, such as ocular surface comprehensive analyzer. 94 patients completed immunoglobulin Ig Gluten Ig Ig M, complement C3, complement C4 and C-reactive protein examination. 78 patients completed anti-nuclear antibody. Detection of anti 偽 lining protein antibody, anti ENA antibody -SSA and anti ENA antibody-SSB. T test and chi-square test were used to compare the relationship between relevant data. Results the subjective questionnaire scores were correlated with corneal fluorescein staining (P0. 019) and lipid layer color (P0. 027). There was a correlation between age and the number of meibomian glands (P0. 022). The color of lipid layer in both eyes (P0. 015), corneal fluorescein staining (P0. 025) and lacrimal river height (P0. 039) were all correlated with serum Ig G level. The level of IgA in serum was correlated with the results of BUT (P0. 022). Conclusion the structure and function of lipid layer of tear film, meibomian gland and cornea are closely related to the ocular symptoms, and the level of immunoglobulin in serum is closely related to the objective examination of ophthalmology. It can be used as an important method to evaluate the whole condition of dry eye and the degree of tear film cornea injury in Sjogren's syndrome.
