【摘要】：目的研究Tolosa-Hunt綜合征(THS)的病因、診斷及治療。方法收集2010年至2014年住院診斷10例Tolosa-Hunt綜合征患者的臨床表現(xiàn)、神經(jīng)影像學(xué)、實驗室檢查及激素治療情況并進(jìn)行回顧性分析。結(jié)果 10例患者的顱神經(jīng)麻痹累及動眼神經(jīng)、滑車神經(jīng)、外展神經(jīng)及三叉神經(jīng)第1、2、3支,有1例為完全性動眼神經(jīng)麻痹。10例患者神經(jīng)影像學(xué)分類均為良性的,應(yīng)用激素后疼痛快速減輕,僅有1例遺留輕度復(fù)視。對治療反應(yīng)無年齡及性別差異。應(yīng)用激素后10例患者疼痛均較眼肌麻痹緩解快。結(jié)論 THS的診斷為排他性,神經(jīng)影像學(xué)對于鑒別診斷THS非常重要,對于THS病人應(yīng)全面完善檢查除外其他疾病引起的疼痛性眼肌麻痹,包括血常規(guī)、紅細(xì)胞沉降率、C反應(yīng)蛋白、甲狀腺、肝腎功能檢查、胸片、頭部及眼眶部CT及磁共振檢查。
[Abstract]:Objective to study the etiology, diagnosis and treatment of Tolosa-Hunt syndrome (THS). Methods from 2010 to 2014, 10 patients with Tolosa-Hunt syndrome were collected and analyzed retrospectively, including clinical manifestation, neuroimaging, laboratory examination and hormone therapy. Results the cranial nerve palsy involved the oculomotor nerve, the trochlear nerve, the abducens nerve and the trigeminal nerve. The pain was relieved quickly after hormone application, and only 1 case had mild diplopia. There was no age or gender difference in response to treatment. The pain relief of 10 patients was faster than that of ophthalmoplegia. Conclusion the diagnosis of THS is exclusive, neuroimaging is very important for differential diagnosis of THS, and should be comprehensive and perfect for THS patients except pain ophthalmoplegia caused by other diseases, including blood routine examination, erythrocyte sedimentation rate and C-reactive protein. Thyroid, liver and kidney function examination, chest film, head and orbit CT and magnetic resonance imaging.
