【摘要】：目的探討低頻重復(fù)經(jīng)顱磁刺激治療耳鳴的有效性和安全性。方法選取聽(tīng)力正常的主觀性耳鳴患者102例,其中顱鳴患者34例(26~65歲,平均53±12.59歲),耳鳴患者68例(31~69歲,平均51±12.29歲);采用1Hz低頻脈沖刺激患者左側(cè)顳頂部,每次一次性連續(xù)給予1 000次脈沖刺激,脈沖間隔時(shí)間為10s,5次/周,連續(xù)2周。所有患者治療前后采用視覺(jué)模擬量表(visual-analogue scale,VAS)評(píng)估耳鳴的嚴(yán)重程度。結(jié)果治療后,34例顱鳴患者中,24例顱鳴減輕或消失,總有效率為70.59%(24/34),其中2例患者自覺(jué)顱鳴完全消失(5.88%,2/34),22例自覺(jué)顱鳴響度減輕(64.71%,22/34),治療前VAS平均分為5.09±1.01分,治療后降為3.06±1.77分;68例耳鳴患者總有效率為64.71%(44/68),44例耳鳴減輕或消失(64.71%,44/68),其中1例耳鳴完全消失(1.47%,1/68),43例自覺(jué)耳鳴減輕(63.24%,43/68);治療前VAS評(píng)分為5.68±1.51分,治療后降為3.75±2.11分。兩組患者治療前后純音聽(tīng)閾無(wú)顯著變化,且無(wú)一例出現(xiàn)頭痛、頭暈、頭皮灼傷、癲癇發(fā)作、耳鳴加重等不良反應(yīng)。結(jié)論低頻重復(fù)經(jīng)顱磁刺激治療耳鳴安全有效,其中顱鳴患者效果更佳。
[Abstract]:Objective to investigate the efficacy and safety of low-frequency repetitive transcranial magnetic stimulation in the treatment of tinnitus. Methods 102 cases of subjective tinnitus with normal hearing were selected, including 34 patients with craniotomy (2665 years old, mean 53 鹵12.59 years old) and 68 patients with tinnitus (31 69 years old, mean 51 鹵12.29 years old). 1 000 pulse stimuli were given at a time of 1 000 pulses at a time interval of 10 sm 5 times per week for 2 consecutive weeks. The severity of tinnitus was evaluated by visual-analogue scale before and after treatment. Results the total effective rate of craniotomy was 70.59% (24 / 34) in 24 of 34 patients with craniotomy after treatment, 2 of them (5.88% / 34) and 22 cases (64.71% 22 / 34) of conscious craniotomy, and the average score of VAS before treatment was 5.09 鹵1.01min, and the total effective rate was 70.59% (24 / 34), and the total effective rate was 5.09 鹵1.01 before treatment. After treatment, the total effective rate of 68 patients with tinnitus was 64.71% (44 / 68), 44 cases with tinnitus were relieved or disappeared (64.71%), one case of tinnitus disappeared completely (1.47% 1.68%) and 43 cases of conscious tinnitus decreased (63.2443% -68), the VAS score before treatment was 5.68 鹵1.51, and after treatment it decreased to 3.75 鹵2.11 points. There was no significant change in pure tone hearing threshold before and after treatment, and no adverse reactions such as headache, dizziness, scalp burn, epileptic attack and aggravated tinnitus were found in the two groups. Conclusion low frequency repetitive transcranial magnetic stimulation is safe and effective in the treatment of tinnitus.
