【摘要】：目的:評價藥物治療無效的開角型青光眼及部分復(fù)雜性青光眼行二氧化碳激光輔助下外層Schlemm管消融術(shù)(CO_2 laser-assisted sclerectomy surgery,CLASS)的短期臨床安全性及有效性。方法:收集2015年5月至2016年1月解放軍總醫(yī)院眼科行CLASS手術(shù)和小梁切除手術(shù)的青光眼患者各23例,共46例46只眼。觀察術(shù)后1周、1個月和3個月眼壓及并發(fā)癥發(fā)生情況。結(jié)果:兩組術(shù)后1周、1個月及3個月眼壓較術(shù)前均明顯下降(P=0.000)。術(shù)后1周、1個月及3個月時CLASS手術(shù)組眼壓平均值均低于小梁切除術(shù)組,但無統(tǒng)計學(xué)差異(P0.05)。CLASS手術(shù)組23例均無淺前房、脈絡(luò)膜脫離等并發(fā)癥出現(xiàn)。小梁切除手術(shù)組有4例在術(shù)后早期出現(xiàn)淺前房,其中1例出現(xiàn)脈絡(luò)膜脫離,經(jīng)治療后均治愈。結(jié)論:CLASS手術(shù)在有效降低眼壓的同時,能大幅降低患者術(shù)后出現(xiàn)淺前房等并發(fā)癥的概率,但其遠期降眼壓效果及并發(fā)癥發(fā)生情況有待進一步深入研究。
[Abstract]:Objective: to evaluate the short-term clinical safety and efficacy of CO _ 2 laser-assisted sclerectomy surgeryCLASS in patients with open angle glaucoma and partially complicated glaucoma treated with CO _ 2 laser-assisted sclerectomy laser assisted with CO _ 2 laser. Methods: from May 2015 to January 2016, 23 glaucoma patients (46 eyes of 46 cases) received class operation and trabeculectomy in PLA General Hospital. Intraocular pressure and complications were observed 1 week, 1 month and 3 months after operation. Results: IOP in both groups was significantly decreased at 1 week, 1 month and 3 months after operation (P0. 000). At 1 week, 1 month and 3 months after operation, IOP in CLASS group was lower than that in trabeculectomy group, but there was no statistical difference (P0.05). There were no superficial anterior chamber, choroidal detachment and other complications in CLASS group. In trabeculectomy group, 4 cases had shallow anterior chamber early after operation, and 1 case had choroidal detachment. All cases were cured after treatment. Conclusion while reducing intraocular pressure effectively, the probability of complications such as shallow anterior chamber after operation can be significantly reduced by the operation of W1 class. However, the long-term effect of reducing IOP and the occurrence of complications need to be further studied. [WT5HZ] [WT5 "BZ] [WT5" HZ]
