【摘要】：目的探討神經(jīng)生長因子(NGF)及生長相關(guān)蛋白(GAP-43)在兒童先天性上斜肌麻痹性斜視發(fā)病中的意義。方法選取28例先天性上斜肌麻痹性斜視患兒行斜視矯正手術(shù)中獲取的病變上斜肌(斜視上斜肌組)15例、下斜肌(斜視下斜肌組)13例,另選取30例因眼外傷行眼球摘取手術(shù)患兒的正常上斜肌(正常上斜肌組)、正常下斜肌(正常下斜肌組)組織各15例。采用HE染色法觀察各組上下斜肌組織病理改變。采用免疫組化染色法檢測各組上下斜肌組織NGF及GAP-43蛋白表達。結(jié)果正常上斜肌組、斜視上斜肌組NGF蛋白陽性表達率為分別為80.0%、46.7%,正常下斜肌組、斜視下斜肌組NGF蛋白陽性表達率分別為73.3%、84.6%,斜視上斜肌組NGF蛋白陽性表達率低于正常上斜肌組,斜視下斜肌組NGF蛋白陽性表達率高于正常下斜肌組(P0.05)。正常上斜肌組、斜視上斜肌組GAP-43蛋白陽性表達率分別為73.3%、33.3%,正常下斜肌組、斜視下斜肌組GAP-43蛋白陽性表達率分別為60.0%、84.6%,斜視上斜肌組GAP-43蛋白陽性表達率低于正常上斜肌組,斜視下斜肌組GAP-43蛋白陽性表達率高于正常下斜肌組(P0.05)。結(jié)論 NGF及GAP-43在先天性上斜肌麻痹性斜視患兒上斜肌中表達降低、在下斜肌中表達增強,二者在先天性上斜肌麻痹性斜視發(fā)病中具有重要作用。
[Abstract]:Objective to investigate the significance of nerve growth factor (NGF) and growth associated protein (GAP-43) in congenital superior oblique paralysis in children. Methods Fifteen children with congenital superior oblique palsy strabismus and 13 patients with inferior oblique muscle were treated with strabismus correction surgery, including 15 patients with superior oblique muscle (superior oblique muscle group) and 13 patients with inferior oblique muscle group. 30 cases of normal superior oblique muscle (normal superior oblique muscle group) and 15 cases of normal inferior oblique muscle (normal inferior oblique muscle group) were selected. The histopathological changes of superior and inferior oblique muscles were observed by HE staining. The expression of NGF and GAP-43 in superior and inferior oblique muscle was detected by immunohistochemical staining. Results the positive expression rates of NGF protein in the normal superior oblique muscle group and the strabismus superior oblique muscle group were 80.0 and 46.7, respectively. The NGF protein positive expression rates in the normal inferior oblique muscle group and the strabismus inferior oblique muscle group were 73.3% and 84.6%, respectively. The NGF protein positive expression rate in the strabismus superior oblique muscle group was lower than that in the normal superior oblique muscle group. The positive expression rate of NGF protein in strabismus inferior oblique muscle group was higher than that in normal inferior oblique muscle group (P0.05). The positive expression rates of GAP-43 protein in normal superior oblique muscle group, superior oblique muscle group and normal inferior oblique muscle group were 73.3 and 33.33.The positive expression rates of GAP-43 protein in normal inferior oblique muscle group and strabismus inferior oblique muscle group were 60.084.6g, respectively, and the positive expression rate of GAP-43 protein was lower in strabismus superior oblique muscle group than that in normal superior oblique muscle group, the positive expression rate of GAP-43 protein in superior oblique muscle group was lower than that in normal superior oblique muscle group. The positive expression rate of GAP-43 protein in strabismus inferior oblique muscle group was higher than that in normal inferior oblique muscle group (P0.05). Conclusion the expression of NGF and GAP-43 in the superior oblique muscle of children with congenital superior oblique palsy strabismus is decreased, and the expression of GAP-43 is increased in the inferior oblique muscle. They play an important role in the pathogenesis of congenital superior oblique paralysis strabismus.
