SD-OCT后極部非對(duì)稱性檢查在青光眼早期診斷中的應(yīng)用價(jià)值

【摘要】:目的:探討頻域光學(xué)相干斷層掃描技術(shù)(spectral-domain optical coherence tomography,SD-OCT)的后極部非對(duì)稱性檢查在青光眼早期診斷中的應(yīng)用價(jià)值。方法:本研究選取2014年9月至2016年10月在我院眼病科就診的早期青光眼患者23例(38眼)為青光眼組,另選取同期在我院健康體檢的正常人20例(27眼)作為對(duì)照組。應(yīng)用頻域光學(xué)相干斷層掃描技術(shù)對(duì)青光眼組和對(duì)照組受試者分別進(jìn)行后極部非對(duì)稱性檢查,對(duì)比分析青光眼組和對(duì)照組后極部地形圖的圖像特征,以及后極部中央?yún)^(qū)、上方內(nèi)環(huán)區(qū)、上方外環(huán)區(qū)、鼻側(cè)內(nèi)環(huán)區(qū)、鼻側(cè)外環(huán)區(qū)、下方內(nèi)環(huán)區(qū)、下方外環(huán)區(qū)、顳側(cè)內(nèi)環(huán)區(qū)和顳側(cè)外環(huán)區(qū)視網(wǎng)膜厚度的變化。同時(shí)對(duì)兩組受試者后極部分別進(jìn)行單眼水平象限非對(duì)稱性分析,并進(jìn)行比較。結(jié)果:1本研究中對(duì)照組及青光眼組后極部均呈現(xiàn)中心凹視網(wǎng)膜最薄、內(nèi)環(huán)較外環(huán)視網(wǎng)膜厚的形態(tài)特征;2青光眼組與對(duì)照組黃斑中心凹1 mm內(nèi)視網(wǎng)膜厚度差異無(wú)統(tǒng)計(jì)學(xué)意義(P=0.308)。青光眼組內(nèi)環(huán)、外環(huán)各象限視網(wǎng)膜厚度均比對(duì)照組變薄,且差異均有統(tǒng)計(jì)學(xué)意義(P0.05);3對(duì)照組后極部單眼水平象限非對(duì)稱性分析結(jié)果為2.0(1.25 3.75),青光眼組后極部單眼水平象限非對(duì)稱性分析結(jié)果為2.0(1.0,3.5),兩組相比較,差異無(wú)統(tǒng)計(jì)學(xué)意義(P=0.35)。結(jié)論:1頻域光學(xué)相干斷層掃描儀的后極部非對(duì)稱性檢查能夠快速、精確的測(cè)量后極部視網(wǎng)膜各參數(shù),同時(shí)能對(duì)結(jié)構(gòu)進(jìn)行自動(dòng)化分析;2其中監(jiān)測(cè)后極部視網(wǎng)膜厚度可以作為青光眼早期診斷及病情追蹤的重要手段;3不支持將后極部單眼水平象限非對(duì)稱性分析作為青光眼早期診斷和評(píng)估病情的指標(biāo)。
[Abstract]:Objective: to evaluate the application of spectral-domain optical coherence tomography-SD-Oct in the early diagnosis of glaucoma. Methods: from September 2014 to October 2016, 23 patients (38 eyes) with early glaucoma were selected as glaucoma group, and 20 healthy persons (27 eyes) were selected as control group. Frequency domain optical coherence tomography (OCTS) was used to examine the posterior pole asymmetry in glaucoma group and control group respectively. The image features of the posterior pole topographic map and the central region of the posterior pole were compared between the glaucoma group and the control group. The changes of retinal thickness in the upper inner ring, superior outer ring, nasal internal ring, nasal lateral outer ring, inferior internal ring, inferior outer ring, temporal internal ring and temporal outer ring. At the same time, the asymmetry of the posterior pole of the two groups was analyzed and compared. Results in this study, the posterior pole of the control group and the glaucoma group showed the thinnest fovea retina, and the retinal thickness of the inner ring was thicker than that of the outer ring. (2) there was no significant difference in retinal thickness within 1 mm fovea between the glaucoma group and the control group (P0. 308). The thickness of the inner ring and outer ring of each quadrant in the glaucoma group was thinner than that in the control group. The difference was statistically significant (P0.05). The results of asymmetric analysis of posterior polar horizontal quadrant were 2.0 (1.25 3.75) in the control group and 2.0 (1.0 鹵3.5) in the glaucoma group. There was no significant difference between the two groups (P0. 35). Conclusion the asymmetry of the posterior pole can be measured quickly and accurately by using the 1 / 1 Frequency-Domain Optical coherence Tomography (OCT), and the parameters of the posterior pole retina can be measured accurately and quickly. It can be used to analyze the structure automatically. Monitoring the thickness of the posterior pole retina can be used as an important means for the early diagnosis of glaucoma and the tracking of the condition. 3. The asymmetric analysis of the horizontal quadrant in the posterior polar region is not supported Early diagnosis and evaluation of the disease by the light eye.
