血清維生素D與2型糖尿病視網(wǎng)膜病變關(guān)系的研究

【摘要】:目的探討2型糖尿病(type 2 diabetes mellitus,T2DM)患者視網(wǎng)膜病變(diabetic retinopathy,DR)與血清維生素D的關(guān)系。方法選取2012年2月至2014年12月期間同時(shí)被納入上海市閔行區(qū)疾病預(yù)防控制中心生物樣本庫(kù)和糖尿病管理系統(tǒng)的2型糖尿病患者310例,根據(jù)眼底檢查分為DR組(n=105例)和正常眼底(non diabetic retinopathy,NDR)組(n=205例),收集患者的基本信息、疾病史和檢驗(yàn)信息,并檢測(cè)各組血清中的25-羥基維生素D[25(OH)D]水平。結(jié)果單因素分析結(jié)果顯示,與NDR組比較,DR組的25(OH)D水平降低(t=3.465,P0.001),25(OH)D缺乏率增高(χ~2=5.313,P=0.021),但是25(OH)D不足率差異無(wú)統(tǒng)計(jì)學(xué)意義(χ~2=0.990,P=0.320)。二元Logistic回歸分析結(jié)果表明,在校正一系列變量(基本特征、疾病史、空腹血糖、餐后血糖、甘油三酯和尿酸)后,25(OH)D水平與T2DM患者并發(fā)DR呈負(fù)相關(guān)(OR=0.954,95%CI:0.926~0.983,P=0.002),25(OH)D缺乏的T2DM患者DR發(fā)病風(fēng)險(xiǎn)是25(OH)D不缺乏者的2.17倍(OR=2.171,95%CI:1.047~4.500,P=0.037)。結(jié)論血清25(OH)D水平與DR的發(fā)生呈負(fù)相關(guān),25(OH)D缺乏可能與T2DM患者DR的發(fā)生有關(guān)。
[Abstract]:Objective to investigate the relationship between retinopathy Dr (Dr) and serum vitamin D in patients with type 2 diabetes mellitus (type 2 diabetes mellitusus T2DM). Methods from February 2012 to December 2014, 310 patients with type 2 diabetes were included in the Center for Disease Control and Prevention of Minhang District, Shanghai, and in the diabetic management system. According to fundus examination, Dr group (n = 105) and normal fundus (non diabetic retinopathy group (n = 205) were divided into two groups. Results the results of univariate analysis showed that the level of 25 (OH) D in Dr group was lower than that in NDR group (t = 3.465 / P 0.001) and the deficiency rate of 25 (OH) D was increased (蠂 ~ (2 / 2) ~ (5.313) P ~ (0.021), but there was no significant difference in 25 (OH) D deficiency rate (蠂 ~ (2 0.990) P ~ (0.320). The results of binary logistic regression analysis showed that in adjusting for a series of variables (basic characteristics, history of disease, fasting blood glucose, postprandial blood sugar), The level of 25 (OH) D in T2DM patients after triglyceride and uric acid was negatively correlated with Dr in T2DM patients (OR0.954 / 95CI: 0.926 / 0.983P0. 002). The risk of Dr in T2DM patients with 25 (OH) D deficiency was 2.17 times as high as that in patients with 25 (OH) D deficiency (OR2.171) 95CI1. Conclusion the level of 25 (OH) D in serum is negatively correlated with the incidence of Dr. It is possible that the deficiency of 25 (OH) D is related to the development of Dr in patients with T2DM.
