【摘要】：目的： 1.研究cVEMP oVEMP和BT的檢測(cè)方法和反應(yīng)特性，探討正常人群的cVEMP、oVEMP和BT是否受性別、年齡和耳間差異的影響； 2．研究BPPV患者的cVEMP、oVEMP和BT的反應(yīng)特性，并通過(guò)與正常人群的各項(xiàng)檢測(cè)數(shù)據(jù)的對(duì)比，探討cVEMP、oVEMP和BT變化對(duì)眩暈定位診斷和預(yù)后評(píng)估的意義。 方法： 對(duì)64例正常人按不同性別、不同年齡和不同耳別分別進(jìn)行cVEMP、oVEMP和BT檢測(cè)。對(duì)33例BPPV患者患耳的cVEMP oVEMP和BT檢測(cè)數(shù)據(jù)分別與健耳和正常人群耳的檢測(cè)數(shù)據(jù)進(jìn)行對(duì)比，分析比較不同組別之間各項(xiàng)檢測(cè)數(shù)據(jù)的差異。 結(jié)果： 1．正常人群組不同性別、左右耳間cVEMP、oVEMP和BT試驗(yàn)的各項(xiàng)檢測(cè)數(shù)據(jù)無(wú)顯著性差異，不同年齡段cVEMP、oVEMP的潛伏期和BT的SPV無(wú)顯著性差異，但不同年齡段cVEMP、oVEMP的峰－峰振幅差異有顯著性意義。 2．病例組患耳的cVEMP、oVEMP各項(xiàng)檢測(cè)數(shù)據(jù)與正常人群耳對(duì)比，差別有顯著性意義。但兩組間BT的SPV無(wú)顯著性差異。 3．病例組患耳和健耳的cVEMP、oVEMP各項(xiàng)檢測(cè)數(shù)據(jù)對(duì)比差異有顯著性意義，，但兩組間BT的SPV無(wú)顯著性差異。 4．病例組患耳的cVEMP、oVEMP異常率分別達(dá)到33%和36%，CP陽(yáng)性率達(dá)到48%。 結(jié)論： 1．正常人群不同年齡段峰－峰振幅差異有顯著性意義，年齡與振幅呈負(fù)相關(guān)，說(shuō)明振幅隨年齡的增長(zhǎng)而降低。 2．病例組患耳的cVEMP、oVEMP各項(xiàng)檢測(cè)數(shù)據(jù)與健耳和正常人群耳的檢測(cè)數(shù)據(jù)對(duì)比差異有顯著性意義，但不同組間BT的SPV無(wú)顯著性差異。病例組患耳的cVEMP、oVEMP異常率分別達(dá)到33%和36%，CP陽(yáng)性率達(dá)到48%。說(shuō)明cVEMP、oVEMP和BT可以作為BPPV前庭功能狀態(tài)的客觀量化評(píng)估指標(biāo)。 3.對(duì)BPPV患者進(jìn)行cVEMP、oVEMP和BT檢測(cè)的結(jié)果表明對(duì)前庭球囊、橢圓囊和半規(guī)管的病理狀態(tài)和功能變化可以獲得客觀評(píng)價(jià)數(shù)據(jù)，提示上述三項(xiàng)檢查方法對(duì)眩暈患者的定位診斷有一定的臨床價(jià)值，進(jìn)而可為眩暈治療的療效和預(yù)后判斷提供客觀依據(jù)。
[Abstract]:Purpose :

1 . To study the detection methods and reaction characteristics of cVEMP oVEMP and BT , and explore whether the cVEMP , oVEMP and BT of normal population are affected by sex , age and ear difference ;


2 . To study the reaction characteristics of cVEMP , oVEMP and BT in patients with BPPV , and to explore the significance of cVEMP , oVEMP and BT changes in diagnosis and prognosis of vertigo by comparing with the detection data of normal population .

Method :

The cVEMP , oVEMP and BT test were performed on 64 healthy subjects with different sex , age and ear respectively . The cVEMP oVEMP and BT test data of 33 patients with BPPV were compared with the detection data of healthy ear and normal ear respectively .

Results :

1 . There was no significant difference in the test data of cVEMP , oVEMP and BT in normal population group . There was no significant difference in the latency of cVEMP and oVEMP in different age groups , but the peak - to - peak amplitude of oVEMP in different age groups was significant .

2 . The cVEMP and oVEMP were significantly different between the two groups , but there was no significant difference between the two groups .

3 . There was a significant difference in cVEMP and oVEMP in the case group and the healthy ear , but there was no significant difference between the two groups .

4 . The abnormal rates of cVEMP and oVEMP were 33 % and 36 % , and the positive rate of CP was 48 % .

Conclusion :

1 . The difference of the peak - to - peak amplitude of the normal population was significant , and the age was negatively correlated with the amplitude , indicating that the amplitude decreased with age .

2 . The cVEMP and oVEMP in the case group were significantly different from those of healthy ear and normal ear , but there was no significant difference between different groups . The abnormal rates of cVEMP and oVEMP in the case group were 33 % and 36 % , and the positive rate of CP reached 48 % . The cVEMP , oVEMP and BT could be used as objective quantitative assessment indexes of the functional status of BPPV vestibular function .

3 . The results of cVEMP , oVEMP and BT test in BPPV patients show that the objective evaluation data can be obtained for the pathological states and function changes of vestibular balloon , oval capsule and semicircular canal . It is suggested that the above three methods can provide an objective basis for the diagnosis of vertigo .
