【摘要】：目的探討透明角膜、鞏膜隧道兩種不同切口白內(nèi)障超聲乳化聯(lián)合人工晶體植入術(shù)后干眼癥發(fā)病情況。方法選取2013年9月—2016年9月于南陽醫(yī)專第一附屬醫(yī)院行白內(nèi)障超聲乳化聯(lián)合人工晶體植入術(shù)200例白內(nèi)障患者,根據(jù)手術(shù)方式不同分為觀察組(采用鞏膜隧道切口,100例)和對照組(采用透明角膜切口,100例)。比較兩組手術(shù)前后干眼癥評分、角膜熒光素染色評分、淚膜破裂時間、基礎(chǔ)淚液分泌量。結(jié)果觀察組術(shù)后1周、術(shù)后1個月干眼癥、角膜熒光素染色評分低于對照組(P0.05)。兩組術(shù)后1周、術(shù)后1個月干眼癥、角膜熒光素染色評分均高于術(shù)前(P0.05)。觀察組術(shù)后1周、術(shù)后1個月淚膜破裂時間、基礎(chǔ)淚液分泌量高于對照組(P0.05)。兩組術(shù)后1周、術(shù)后1個月淚膜破裂時間、基礎(chǔ)淚液分泌量均低于術(shù)前(P0.05)。結(jié)論鞏膜隧道切口在白內(nèi)障超聲乳化聯(lián)合人工晶體植入術(shù)的應(yīng)用中相較于透明角膜在術(shù)后1個月內(nèi)對淚膜的穩(wěn)定性影響較輕,顯著降低干眼癥癥狀。
[Abstract]:Objective to investigate the incidence of dry eye after phacoemulsification and intraocular lens implantation in two different incisions of transparent cornea and scleral tunnel. Methods from September 2013 to September 2016, 200 patients with cataract underwent phacoemulsification combined with intraocular lens implantation in the first affiliated Hospital of Nanyang Medical College. The patients were divided into observation group (100 cases with scleral tunnel incision) and control group (100 cases with transparent corneal incision). The dry eye score, corneal fluorescein staining score, tear film rupture time and basal tear secretion were compared between the two groups before and after operation. Results the corneal fluorescein staining score in the observation group was lower than that in the control group at 1 week and 1 month postoperatively. The corneal fluorescein staining score in both groups was higher than that in preoperation (P 0.05). The time of tear film rupture in the observation group was higher than that in the control group at 1 week and 1 month after operation, and the amount of basal tear secretion was higher than that in the control group (P 0.05). The time of tear film rupture and the amount of basal tear secretion in both groups were lower than those before operation (P 0.05) 1 week after operation and 1 month after operation. Conclusion the application of scleral tunnel incision in cataract phacoemulsification combined with intraocular lens implantation has less effect on the stability of tear film than that of transparent cornea within one month after operation.
