【摘要】：目的:評估不同療程標準化粉塵螨滴劑舌下含服特異性免疫治療(SLIT)兒童過敏性哮喘(AS)伴鼻炎的療效和安全性。方法:回顧性分析158例4~14歲AS伴鼻炎患兒接受粉塵螨滴劑舌下免疫治療的臨床指標和病例資料,根據(jù)SLIT療程的長短被分成4組(1年、2年、3年和4年組),接受定期的臨床觀察及隨訪。評估4組患兒治療前后的哮喘日間、夜間癥狀評分、哮喘用藥評分、鼻炎癥狀評分和鼻炎用藥評分的變化。同時記錄治療期間出現(xiàn)的不良反應。結果:與治療前相比,1年、2年、3年以及4年療程患兒的哮喘日間、夜間癥狀評分、哮喘用藥評分、總鼻炎癥狀評分、鼻炎用藥評分均顯著下降,差異有統(tǒng)計學意義(P0.01);4年療程組患兒的哮喘藥物評分相較1年、2年和3年療程組更低,差異均有統(tǒng)計學意義(P均0.05);3年組的鼻炎用藥評分顯著低于2年組(P0.05)。4組患兒的哮喘日間癥狀評分、哮喘夜間癥狀評分、總鼻炎癥狀評分的差異均無統(tǒng)計學意義(P0.05)。結論:通過觀察分別接受1~4年不同療程的SLIT的AS伴AR患兒,發(fā)現(xiàn)不同療程標準化粉塵螨滴劑SLIT對塵螨過敏的患兒均具有顯著療效,且4年療程療效最佳。
[Abstract]:Objective: to evaluate the efficacy and safety of sublingual immunotherapy for allergic asthma with rhinitis in children with allergic asthma. Methods: the clinical indexes and data of 158 patients with as with rhinitis who received sublingual immunotherapy with Dermatophagoides farinae were retrospectively analyzed. The patients were divided into 4 groups according to the length of the SLIT course (1, 2, 3 and 4 years) for regular clinical observation and follow-up. The changes of daytime, nocturnal symptom score, asthma medication score, rhinitis symptom score and rhinitis drug use score were evaluated before and after treatment. Adverse reactions during treatment were also recorded. Results: compared with before treatment, the scores of daytime, night symptom, drug use of asthma, total rhinitis and rhinitis were significantly decreased in children with asthma for 1 year, 2 years, 3 years and 4 years of treatment. The score of asthmatic drugs in the 4-year treatment group was lower than that in the 1-year, 2-year and 3-year treatment groups. The score of rhinitis in 3 years group was significantly lower than that in 2 years group (P 0.054.The score of asthma symptom at night and the score of total rhinitis symptom were not statistically significant). Conclusion: by observing the children with AR who received different courses of SLIT for 1 ~ 4 years, it was found that different courses of standardized slit had significant curative effect on children with dust mite hypersensitivity, and the best therapeutic effect was in 4 years.
