【摘要】：目的:探討半規(guī)管阻塞術(shù)對(duì)3、4期頑固性梅尼埃病的療效。方法:29例梅尼埃病患者術(shù)前聽力分期為梅尼埃病3、4期,其中3期12例,4期17例,均行半規(guī)管阻塞術(shù)。對(duì)比分析患者術(shù)前與術(shù)后眩暈、聽力、前庭雙溫試驗(yàn)、耳鳴分級(jí)、功能分級(jí)等情況。隨訪6~23個(gè)月,平均13個(gè)月。結(jié)果:術(shù)后眩暈控制率100%,其中26例(89.7%)完全控制,3例(10.3%)基本控制;術(shù)后聽力無明顯變化者25例(86.2%),聽力提高2例(6.9%),聽力下降2例(6.9%);前庭雙溫試驗(yàn)示術(shù)后術(shù)耳半規(guī)管功能減弱;術(shù)后耳鳴改善12例(41.4%);功能分級(jí)下降28例(96.6%)。結(jié)論:半規(guī)管阻塞術(shù)能有效控制3、4期梅尼埃病患者的眩暈發(fā)作,短期療效確切,能有效改善患者生活功能,但長期療效仍需繼續(xù)觀察。
[Abstract]:Objective: to investigate the effect of semicircular canal occlusion on refractory Meniere disease in stage 3 and 4. Methods Twenty nine patients with Meniere's disease were divided into three stages and four stages before operation, including 12 patients with stage 3 and 17 patients with stage 4. All patients underwent semicircular canal occlusion. The vertigo, hearing, vestibular double-temperature test, tinnitus grade and functional grade were compared and analyzed before and after operation. The follow-up ranged from 6 to 23 months (mean 13 months). Results: the control rate of postoperative vertigo was 100, of which 26 cases (89.7%) were completely controlled (10.3%), 25 cases had no obvious change of hearing after operation (86.2%), 2 cases improved hearing (6.9%), 2 cases had hearing loss (6.9%), the vestibular double temperature test showed that the function of ear semicircular canal was weakened after operation. Postoperative tinnitus was improved in 12 cases and functional grade was decreased in 28 cases. Conclusion: semicircular canal occlusion can effectively control vertigo attack in patients with Meniere's disease in stage 3 and 4. The short-term curative effect is definite and the life function of patients can be improved effectively, but the long-term curative effect still needs to be observed.
