【摘要】：目的:觀察阻塞性睡眠呼吸暫停低通氣綜合征(OSAS)相關性高血壓患者血清胱抑素C(cystatine,CysC)和超氧化物歧化酶(Superoxide dismutase,SOD)水平的變化。方法:收集2013-01至2013-12在甘肅省人民醫(yī)院心內科和鼾病科就診并完成多導睡眠監(jiān)測的高血壓患者102例,根據呼吸暫停低通氣指數(Apnea hypopnea index,AHI)將102例高血壓患者分成2組:AHI≥5次/h的納入OSAS相關性高血壓組,即OH組55例,AHI5次/h的納入單純高血壓組,即高血壓組47例。另選取同時間段內完成多導睡眠監(jiān)測的健康人45名作為對照組。對所有研究對象進行一般情況、生化指標、睡眠參數方面數據的整理,采用SPSS17.0軟件分析并比較3組上述指標間差異是否有統(tǒng)計學意義。結果:與對照組比較,OH組、高血壓組的血清CysC水平[(1.06±0.27)mg/L和(0.91±0.19)mg/L vs(0.79±0.32)mg/L]明顯升高,血清SOD水平[(127.63±63.19)U/ml和(149.14±60.12)U/ml vs(181.54±59.37)U/ml]明顯下降,組間比較差異均有統(tǒng)計學意義(P均0.05)。進一步將OH組按OSAS病情分為輕度者、中度者、重度者3個亞組,并將3個亞組結果進行比較可見,血清CysC、SOD在輕度和中度、輕度和重度以及中度和重度間比較,差異均無統(tǒng)計學意義(P均0.05)。結論:高血壓組和OH組血清CysC水平高于對照組,而血清SOD水平低于對照組;且OH組血清CysC水平高于高血壓組,血清SOD低于高血壓組(P0.05),但其水平變化與OSAS病情程度的關系尚未證實。
[Abstract]:Aim: to observe the changes of serum cystatin (CysC) and superoxide dismutase (SOD) levels in patients with obstructive sleep apnea hypopnea syndrome (OSAS) associated hypertension. Methods: 102 patients with hypertension who were admitted to Department of Cardiology and snoring Department of Gansu Provincial people's Hospital from January to December 2013 and completed polysomnography were collected. According to apnea hypopnea index (AHI), 102 patients with hypertension were divided into two groups: OSAS group (n = 55) and hypertension group (n = 47). Another 45 healthy people who completed polysomnography in the same time period were selected as control group. The data of biochemical indexes and sleep parameters were analyzed and compared by SPSS17.0 software. Results: compared with the control group, the serum CysC levels [1.06 鹵0.27)mg/L and 0.91 鹵0.19)mg/L vs(0.79 鹵0.32)mg/L] were significantly increased, and the serum SOD levels [127.63 鹵63.19)U/ml and 149.14 鹵60.12)U/ml vs(181.54 鹵59.37)U/ml] were significantly decreased in the hypertensive group (P < 0.05). The OH group was further divided into 3 subgroups according to the condition of OSAS: mild group, moderate group and severe subgroup. The results of the three subgroups were compared. Serum CysCnsSOD was compared between mild and moderate, mild and severe, moderate and severe. The difference was not statistically significant (P = 0.05). Conclusion: the serum CysC level in hypertension group and OH group is higher than that in control group, while the serum SOD level in OH group is lower than that in control group, and the serum CysC level in OH group is higher than that in hypertension group. The level of serum SOD was lower than that of hypertension group (P 0.05), but the relationship between the level of serum SOD and the severity of OSAS was not confirmed.
