【摘要】：目的:探討術(shù)前外周血淋巴細(xì)胞單核細(xì)胞比值(LMR)在區(qū)分喉鱗狀細(xì)胞癌(LSCC)、喉癌前病變(LPL)以及喉良性病變(LBL)中的作用。方法:收集405例經(jīng)病理確診的喉部病變患者(LSCC組154例,LPL組83例,LBL組168例),LSCC組按T分期進(jìn)一步分為T1組、T2組和T3~4組,檢測各組術(shù)前外周血中性粒細(xì)胞、淋巴細(xì)胞、單核細(xì)胞、血紅蛋白和血小板等計(jì)數(shù),并比較各組中LMR、中性粒細(xì)胞淋巴細(xì)胞比值(NLR)及其他指標(biāo)。結(jié)果:LBL、LPL、LSCC各組中LMR和NLR值分別為4.88±1.65和4.61±1.59、3.92±1.77和2.11±0.79、2.08±0.77和2.63±1.69,LSCC組LMR較LBL組和LPL組小,且差異有統(tǒng)計(jì)學(xué)意義(P0.01),而NLR較LBL組和LPL組大(P0.05),LBL組和LPL組之間LMR和NLR差異均無統(tǒng)計(jì)學(xué)意義(P0.05)。LSCC組中局部晚期(T3~4期)患者較局部早、中期(T1、T2期)患者LMR小,而NLR值大且差別具有統(tǒng)計(jì)學(xué)意義(P0.05)。LBL、LPL、LSCC三組的血小板、白細(xì)胞、中性粒細(xì)胞數(shù)之間差異無統(tǒng)計(jì)學(xué)意義(P0.05)。結(jié)論:本研究首次比較不同喉部病變中的LMR,并發(fā)現(xiàn)LMR、NLR作為全身性的炎癥指標(biāo)對區(qū)分LSCC和LPL及LBL具有一定的參考價(jià)值。
[Abstract]:Objective: to investigate the role of LMRs in distinguishing LSCC, LPLs and LBLs in laryngeal squamous cell carcinoma (LSCC), precancerous laryngeal lesion (LPL) and benign laryngeal lesion (LBL) before operation. Methods: a total of 405 patients with laryngeal lesions diagnosed by pathology were collected. 154 patients in LPL group and 83 patients in LBL group were divided into T _ 1 group and T _ 2 group according to T staging. The peripheral blood neutrophils, lymphocytes and monocytes were detected in each group before operation. Hemoglobin and platelet count were compared and LMRs, neutrophil lymphocyte ratio (NLR) and other indexes were compared. Results the values of LMR and NLR were 4.88 鹵1.65 and 4.61 鹵1.59 鹵3.92 鹵1.77 and 2.11 鹵0.79 鹵2.08 鹵0.77 and 2.63 鹵1.69 in the LSCC group respectively, and the LMR values in the LSCC group were lower than those in the LBL group and LPL group. There was no significant difference in LMR and NLR between LBL group and LPL group. There was no significant difference in LMR and NLR between LBL group and LPL group. There was no significant difference in the number of platelets, leukocytes and neutrophils among the three groups. Conclusion: LMRs in different laryngeal lesions were compared for the first time in this study, and it was found that LMR-NLR was useful in differentiating LSCC from LPL and LBL.
